The product, in the report: Rainbow Play systems King Kong Castle Swing. Sling Swing Failure

On May 20, 2015 my son was swinging on our King Kong Castle swing set when the sling swing failed breaking through the grommet as well as the sling swing. He fell while swinging hitting his head, back and bottom/tailbone hard on the ground as he was swinging. He was injured with a head wound on the back of his head requiring 8 staples. He has complained of neck, jaw soreness, as well as pain on sitting and is having headaches as a result of a concussion. He has memory loss of the incident as well as the hospital visit. The failure of the sling swing was at the attachment point of the chain which ripped through the grommet and swing.
