Eastern Jungle Gym Easy 1-2-3 A-Frame Swing Set Brackets were purchased and combined with locally acquired lumber and an unknown tire swing. It was installed, unanchored and over bare earth, at a preschool.
Approximately two weeks after installation, three preschool-age children were playing on the swing when the whole frame fell over. Two children were uninjured, but the third suffered a head injury and died later that day.
