Emergency room Made Jan 16, 2015, sent to the company Jan 27, 2015, published by CPSC Feb 17, 2015
The product, in the report: Adventure Playsets Pathfinder wooden swing set
My 12 year-old daughter (approx. 80-90 lbs.) was swinging on our Adventure Playsets Pathfinder swingset recently when one side of the swing’s caribiner-style link failed, sending her to the emergency room with a concussion. Upon inspection afterwards, the caribiner had worn completely through both its shaft and the “protective” nylon bushing, and wear was also shown on the top pivot. These swingsets, being made primarily of wood, require periodic inspection and attention, which for me also included an inspection of the metal and plastic parts. I do not recall noting any wear on the caribiner itself or the nylon bushing upon my last inspection within the last few months, other than surface rust around the threaded portion (which was not involved or otherwise a factor in the break). Although the swingset is about six or seven years old and is exposed to seasonal changes here in Southern Ohio, I would have expected that it would last long enough for one child’s use and also perhaps our four year-old son’s use, and not to be such an unsafe product.

The product, in the report: Adventure Playset The Sherwood Wooden Playcenter Model 1-AP049-07
We have an Adventure Playsets The Sherwood Wooden Playcenter, model 1-AP049-07. The monkey bar beams have already been replaced professionally under recall, and these are the beams that failed.
My 6-year-old son was swinging on one of the swings, and we heard a loud crack. Fortunately, we were able to get him off the play set before an emergency situation occurred. The crack is visible, right at the screw attaching the beam to the slide/playhouse.

No one hurt Made May 2, 2012, sent to the company May 10, 2012, published by CPSC May 24, 2012
The product, in the report: Play set has green and yellow canopy over top with a green one down lower. It has monkey bars on one side and two single and a double swing on the other. It has a picnic table area under the green canopy and a play house under the slide platform. I have tried to add pictures a couple of times but after I put the descriptions on they won’t attatch. Tried other ways to..I could email through my [REDACTED] account I have 17 pictures.
I have an Adventure wooden swing set that is not in the recall but the wood is breaking and rotting on this one as well, it is the Meridian set. I was questioning the safety of it this spring while my daughter played on it. The swings seem to be a little loose up top as well. I can send pictures of the damage to this play set I bought and assembled in the summer of 2009. The monkey bars area is especially bad where it is cracking and coming apart. No injuries have occurred yet.

The product, in the report: Swing set with two sling-style seats, a slide, and a climbing wall.
I heard of a [REDACTED] recall on the [REDACTED] sling-style swing seats. A couple of years ago I bought in WalMart a similar playground equipment (Adventure Playsets) with the same seats style. One of the seats is half cracked. So far, nothing has happened, but I’d like to know if there any way we could know of the equipment being recalled as well. Do you know how we can replace the seats?

The product, in the report: Yukon II Playset Features Item# 1030 features: English Full featured playset with all the goodies for a very low price Fabric canopy cover on the upper deck 8’ Super Safe Speedy Slide 2 swings and trapeze bar Rung ladder and rock wall ladder Picnic table Angled frame for maximum stability Upper deck dimensions: 55” x 25” Upper deck height: 4’
We have purchase an Adventure Playset a little more then a year ago. It was all installed correctly as per the instructions. However, we found that the “A” frame that extand from the fort and holds the swing set is starting to split and possibly will break which will cause and injury if a child is on it. Weather is also seem to effect it.
I looked at recalled products and see that the one we have doesnt appear to be on it. However, I believe that it posses a very series risk to children.
I cannot find the manual so I cant give teh exact number.

The product, in the report: Adventure swing set Madison 1- AP006
I have the Adventure Swing set, the Madison 1- AP006....we have had the pieces replaced that were involved in the recall....but the set is shifting like crazy. Two of the upright posts (also coated in plastic) have caps that opened up letting water into them, and my father who is a mechanic went to tighten the bolts, and found that some of the bolts on the main structure have broke in half. I no longer feel that this play set is safe for my young children to use.

No one hurt Made Jun 17, 2011, published by CPSC Jul 19, 2011
The product, in the report: swing set
I have an adventur playset atlantis model. The set is falling splitting, splintering, and breaking. Handle used for getting up stairs on the unit broke off while my 4 year old Down Syndrome daughter was climbing it. I was able to catch her in time before she hurt herself. The main beam holding the swings is splitting and the set is loose causing the whole set to sway when she swings.
I contact the manufactures customer service and they told me basically there was nothing they would do. I had to pay for the replacement parts. I contact Walmart at the advice of the manufacture and Walmart says it was past 90 days.

No one hurt Made May 25, 2011, published by CPSC Jun 17, 2011
The product, in the report: Wooden playset for children that has monkey bar/swings and a component with a slide and ladders.
We have the Tacoma Playset that was mentioned in a previous safety recall, http://www.cpsc.gov/cpscpub/prerel/prhtml10/10029.html . We received the new monkey bar set to fix the playset. The problem is that the same defect that occurred in the previous recall (the rotting wood surrounded by the plastic coating) happened in the other part of the playset (the part with the ladders and slide). So, when the children swing, the entire playset becomes unstable, and the monkey bar and slide sections are becoming detached due to the weak supports on the slide side.
