First aid Made Mar 8, 2016, sent to the company Mar 16, 2016, published by CPSC Apr 6, 2016
The product, in the report: Rainbow Monster Play system
One of the Yellow plastic trapeze rings on our Rainbow Play System cracked and broke off while in use by our daughter resulting in injury. Fortunately, and due to a properly mulched playground the injury was not life threatening. These are the same rings covered by the recall “Recall number: 15-250” with recall details listed as “Rainbow dealers nationwide from January 2007 through December 2011 and at several mass merchandisers including Sam’s Club, Toys R Us and Walmart from January 2009 through December 2009. The playsets retailed for between $900 and $10,000”. However, the recall only covers those systems purchased from Rainbow dealers from 2007 to 2011. The set we purchased was in 2005. We request this recall be expanded to cover all years using the same plastic rings manufactured & Engineered with negligence, We request in lieu of a $10 Amazon gift card, Rainbow replace the plastic rings with plastic coated metal rings which can be obtained on Amazon for just under $30. We have already contacted the manufacturer and they refused to cover their negligence in manufacturing & design as per the message below. There may be thousands of additional Rainbow Systems out there beyond the recall period which pose a serious risk to children up to potentially life threatening head and neck injuries. We also request Rainbow be forced to send notification to all owners covered under the expanded recall.
Hi [REDACTED],
Unfortunately this was the agreement between Rainbow and the CPSC. The only option that we have is to issue the $10 Amazon Gift Card. It was an oversight on my end and I shouldn’t have provided the $10 Amazon Gift Card because your set was purchased in 2005.
Thank-you!

No one hurt Made Mar 18, 2013, sent to the company Apr 15, 2013, published by CPSC Apr 29, 2013
The product, in the report: Trapeze handle found on [Sunshine Castle PKG II] of [Rainbow Play Systems] outdoor play equipment.
Son (8 years old, ~64lbs) was hanging on the trapeze bar of outdoor play equipment purchased by Superior Play Systems in Bear, DE. The yellow trapeze handle broke apart causing my son to fall to the ground. He was not injured. Fortunately, he was just hanging on the handle right side up (not upside down).
I’m reporting this because I consider this trapeze handle to be a safety risk for children. If a child is upside down while hanging on these trapeze handles and it breaks, they could fall to the ground on their head and break their neck.

Hurt, no treatment Made Apr 27, 2012, sent to the company May 7, 2012, published by CPSC May 21, 2012
The product, in the report: Rainbow Double Decker as sold through Costco Spring 2009 childrens play set for ages 3-11
Rainbow Systems Play set - While my 9 year old daughter was playing on her play set the plastic rigs which are part of the “10J Trapeze/Rings combo” accessory broke off. My daughter fell down and was not seriously injured, but did get a bruise on her leg from the fall. http://www.rainbowplay.com/index.php/swingsets/options/C140/# Rainbow systems asked me to send them a picture of the broken part, which I did, and they have sent me a replacement pair of rings. The original rings were a molded plastic set, and the new rings are cast aluminum. This is a pretty significant redesign, most likely due to other rings failing in the same way. When I looked for a recall on this, I was shocked to not see one. I’m guessing that mine was not the first problem they had, and wonder how many other sets of defective rings are in consumers hands, only to be noticed after someone gets injured. Hopefully you can look into this and get these replaced before someone gets killed or paralyzed.
