The product, in the report: Jetson Bolt Electric Bike. It was black.
The Jetson Bolt was being ridden on a flat, well paved bike path. The front wheel locked and became impossible to move and impossible to stop the bike. I was thrown over the handle bars, receiving severe injuries that required hospitalization.

The product, in the report: Jetson Bolt Folding E-Bike Full Throttle Electric Bicycle with LCD Display, Brand Jetson Electric Bike Color Black Handlebar Type Foldable Wheel Size 12 Inches Frame Material Aluminum Item Weight 34 Pounds Suspension Dual Brake Style Rear Braking
After 5 minutes riding the Jetson Bolt Electric bike, I was thrown over the handlebars and ended up face down knocked unconscious. My helmet was useless because I landed face down. I sustained a concussion, and required an emergency room visit. I injured my knee as well and it has still not healed. My black eye is finally disappearing. I bought the unit because it was compact and I could take it to [REDACTED] easily and could use it on the compacted sand at the beach, where 100’s of bikes are on the beach at any time. . The box gave it a weight max of 265. I am 240. Their website now lists max of 250. I bought it at Costco.

Hurt, no treatment Made May 12, 2020, sent to the company Jun 26, 2020, published by CPSC Jul 20, 2020
The product, in the report: electric bicycle
The consumer is concern about the safety of the product because of the way the unit is built and the firm is aware of it..
The consumer experienced three incidents with the product.
On 5/9 the consumer got on the bike and it rode out from under him because he was sitting too far back on the bike.
The consumer stated that the wheels are really close together and really small. The platform is also unstable.
On 5/10, the consumer took the bike out on the street to ride. When he went to make a turn, the limiter allowed the wheels to turn too far. The wheels turned sideways and he was thrown off of the bike.
On the same day, he took it over to a younger fellow so that he could ride it to see if it was just him that was having an issue. When he transitioned from the street to the driveway, he hit the driveway lift and the back of the bike flipped over. The consumer fell over on his face.

The product, in the report: Jetson Bolt E-bike. https://ridejetson.com/products/bolt-electric-bike
Jetson Bolt ebike has a single brake. It is intended for light duty and relatively low grade inclines. However I am concerned that single brake (no redundant braking) is unsafe on downhill grades. I was on a hill using a lightly used (basically new) Bolt ebike. I was going down hill. The brake was unable to slow the decent and the bike continued to increase in speed. I was fortunate that half way down the hill, there was a fork and one of the paths went up hill (like a truck runout). I was not injured but it was terrifying. Reviewers of the bike noted problems with braking and expressed concern over its performance. (I won the bike at a workplace event.)
Note, my weight is equal to or slightly below the maximum weight rating for the bike. So fully loaded on a steeper grade.
I raised the issue with the company over facebook. They were responsive but I question whether they were really invested in investigating the matter. They stated that would look in to it, but did not ask questions, did not request contact information or have any desire to make arrangements to inspect the bike when I offered it. It was unclear whether they would follow up. They appeared to consider this more of a user error. I don’t know -- it’s in the factory configuration, has traveled maybe 2KM over its life, and should be in good mechanical order.
