The product, in the report: Slow Cooker Sensio WJ-5000de manufactured date code 0909
Slow cooker set to low, left house for day trip and returned three hours later. When we returned the slow cooker was not operating, and on closer inspection the bottom of the control panel was melted. There was evidence of smoke around the perimeter of the control panel. When I typed in the model number to look for what size to replace the slow cooker with, I saw there was a recall already in place.

The product, in the report: 5 qt. 3 setting Slow Cooker
Consumer started to cook a roast in the slow cooker on the low setting and placed the lid on. Within 5 minutes the glass lid shattered and the handle on the lid flew off. The lid still remained on the cooker, but it was shattered. There is a 3 inch wide hole where the handle once was secured to the lid. Some of the pieces of glass near the handle also separated from the rest of the glass.
After cleaning the kitchen twice to remove the fragments of glass; the consumer’s daughter [redacted] , sustained minor cuts to both hands by untraceable pieces of glass. The cuts on her right wrist and left thumb were rinsed off and bandaged; no medical attention was needed. Consumer has only used the slow cooker about 4 times, since receiving it as a gift before the holidays. There were no known cracks or blemishes on the slow cooker.
On the same day the consumer spoke with mfr ([redacted] operator#[REDACTED]) who stated she could send a replacement lid and it would be referred to the Claims Mgr [redacted]. The consumer did not indicate whether or not he would accept the replacement lid. He was more interested in speaking with [redacted]. The consumer was told that [redacted] would call him back at a later time.
