Made Jul 28, 2015, sent to the company Aug 5, 2015, published by CPSC Aug 19, 2015

The product, in the report: Proctor-Silex slow cooker. Type SC19; Model number: 33275Y; series number: E0331DJ.

Today, July 26, 2015, the glass lid on our Proctor-Silex slow cooker exploded. The handle on the lid hit the bottom edge of the microwave (about one foot away to the left) and partially dislodged a filter screen, then landed on the counter top nearby, to the right of the slow cooker. The glass lid is completely shattered. The pot had not boiled dry, there was plenty of liquid.

No one was nearby, so no injuries were sustained, just lost a pot of beans!

Had anyone been close to the slow cooker when the top exploded, serious injury could have been sustained.

The model number is 33275Y; series number is E0331DJ.

The product, in the report: Slow cooker 3 qt oval shape, glass lid, lift out ceramic insert.

This is about the slow cooker brand Proctor Silex.

Monday, February 24th, 2014 I cooked chicken in my Proctor Silex [slow cooker] for 10 hours on Low. Food was good. Everything was good. Our dishwasher person forgot to wash the lid that night and it was left in the kitchen on a table until Tuesday, February 25th, 2014.

Tuesday we had our person dishwasher wash the lid by hand. Upon inspection Wednesday it was discovered still a little dirty.

Saturday, it was re-washed by hand (using only liquid dish detergent, warm water, and a wash cloth). Upon inspection Sunday morning it appeared still a tiny bit dirty. Our person washer RE-WASHED it Sunday morning and stood it to dry in the dish drainer/rack with other clean plastic dishes. That was 8:30am.

Sunday 12:30pm on March 2nd, 2014 when we returned home from church, the lid was still standing in the drying rack but the glass had completely shattered. Pieces were everywhere in the clean dishes, in the sink, and on the floor. I was concerned because we have 5 children that all entered the kitchen not knowing what we were walking in to. We also have a puppy that we’re raising for the Seeing Eye and I was worried she’d get glass in her paws.
