The product, in the report: Follett 2013 Ice Machine Model: 12HI 400 A
The Ice machine in the ICU started to smoke. The fire department and facilities management were notified. The Ice machine was taken out of service and the manufacturer was notified. No one was harmed and this did not disrupt patient care. The Follett representative was contacted and stated that due to the previous ice machine fire they investigated, they found an issue with the relay and were in process of a recall to replace these defective part. They started getting other facilities that were having the same issue. The root cause of the problem was that GE did not use enough copper wire in the windings of the relay when making the part, causing them to over heat and melt. The ICU Ice machine was replaced, and 3 other ice machines had the defective part replaced.
