The product, in the report: Gypcrete is a cheaper concrete sub floor substitute made of gypsum substances

On or about September, 2015, complainant, a disabled polio victim, noticed gypcrete subflooring prodigious disintegration producing an uneven soft surface in that chunks of the imitation concrete occurred and caused prodigious dust, causing coughing and sneezing and breathing difficulties. The floor lacks the durability needed to sustain foot traffic and disability equipment, including wheelchair, walker, crutches and point loads from furniture and appliances. Multiple hazardous incidents occurred, including but not limited to falling, orthopedic, muscular injuries, breathing difficulty, and intractable pain and sleep interference. Access to water, appliances, bathroom, bedroom, and kitchen requires hazardous abnormal “acrobatic” movements, especially in relation to oven safety protocol. Aforementioned multiple continuous incident prompted this report, as injuries cause death. This product is hazard to persons protected by the Americans with Disabilities Act. Complainant’s service dog has suffered hip injury and cannot provide optimal assistance. Refer to attachments 1 (floor photo) and 2 (complainant’s disability status
