No one hurt Made Oct 28, 2019, sent to the company Jan 28, 2020, published by CPSC Feb 11, 2020
The product, in the report: Cosco 3 N 1 Hybrid Convertible Hand Truck 12326ASB Unit. Manufactured July 16, 2016, Model # 12-326 ASB. Purchased at Costco, Roseville, MI. UPC 0 44681 12148 7
I was using a Cosco Hand cart 3-n-1(12326ASB) and transforming from two-wheel to four-wheel usage.
The four rivets holding the handle to the frame broke away from the frame causing a potentially dangerous situation. The cart cannot be used in its present condition. It would seem a more robust way of attaching the handle is in order. The cart was not being used at this time and I was simply trying to transform from two to four wheel use. The cart has a capacity of 800 lbs. I can’t imagine if I would have had something on the cart and moving an object up stairs. I set the cart aside and contacted Cosco of the product failure twice.
I have seen that a similar [REDACTED] from Cosco was [REDACTED] on a different issue. I have advised Cosco twice of the incident and have sent pictures per their request.

The product, in the report: Cosco Brand. Model 12-301 ABL
I was putting air into the tire on this unit. The tire suddenly blew off- like a cannon ball. I was not injured, but the flying tire was inches from my face. The tire rubber split and the tube was popped. The metal hub cap was damaged as the lug nuts pulled right thru the 4 holes. The hub cap went flying also.

Emergency room Made Jun 15, 2018, sent to the company Jun 25, 2018, published by CPSC Jul 10, 2018
The product, in the report: Cosco Hand Truck Model #12-326 ASB it’s a gray/silver convertible 3-in-1 hand cart
This hand truck is a convertible cart which can be used 3 different ways. It was in the position where the back of the cart is at an angle. This is the assisted 4-wheel position. My child was moving it over when he lost his footing. He grabbed the cart from the front to catch himself, when it gave away folding shut. Both the ring and pinkie fingers on the left and right hand were trapped inside the sliding railing mechanisms. The latch had to be lifted to allow the cart to open so the hands could be pulled out. The fingers were all disfigured and bleeding. They were flattened so much I thought they had been sliced off. By the time we got to the Emergency Room, they had begun to plump back up and looked much better. The right hand was not injured as badly as the left hand. He had an appointment with the hand orthopedic specialist on 6-11-2018. At this appointment the Dr. told us he would have surgery to repair both the left ring and pinkie fingers. His surgery was on 6-12-2018. After he had the surgery and was getting better, I went to check out the hand cart to try and figure out what had happened. I was able to convert it between all the positions with no problem so I just assumed I didn’t have it latched properly when the injury happened. Then on 6-14-2018 I was moving a shipment of boxes I received from FedEx when it slammed shut again. Thankfully there were no injuries this time. I once again started to question myself, did I latch it? I thought I made sure it was latched. It was during this check that I found out the latch was in the correct position but was not fully securing the plastic hook part of the latch around the metal tubing that it’s suppose to secure to. I tried it several times and sometimes it worked and other times it didn’t. I found out that I had to force the latch down and then it stayed latched. The second latch seems to be working great, I’ve had no problems with it, only the latch for the 4-wheel assisted position.

First aid Made May 31, 2017, sent to the company Jun 8, 2017, published by CPSC Jun 22, 2017
The product, in the report: Aluminum 3 in 1 hand truck. (Upright, leaning at an angle, or flat in a cart position.) Upper handle assists in the flat cart position. Lower handle assists in the angle position and locks the extra wheels away in the upright position with no load. See video [REDACTED]
In the month of April, 2017 I acquired a second hand aluminum hand truck, Costo Mod. 12-302 ABL 3in1 (multi position). One of the handles was broken so I contacted Costco by email 3 times to get the “handle” (safety bracket) replaced. They sent me the wrong handle 3 times, the upper one, when I insisted I needed the lower handle. The Lower handle supports 2 positions. Regular upright, where the hand truck is in an upright position as any other handtruck would be [handle carries 0 load]. Also, the lower handle can be unlocked and 2 extra wheels lower making the truck roll at an angle (leaning back) carrying the FULL load for the human [that part of the handle was operational]. This truck is labeled for 1000lbs upright and 800lbs leaning back on the extra 2 small wheels (4 wheels total). Since I was having hardships carrying a FULL load of sodas or plastic water cases where the load was being carried by my arms & back... the leaning position was the excellent solution for me. The lower broken handle was fully operational in that SINGLE position. I wrote Costco CS, and like I said, they sent me the wrong bracket 3 times despite my excellent communication capabilities. On 2017/05/24 (May 24th) the hollow plastic bracket broke off and the 5 cases of water, each 5.25 gal. were sent to the sidewalk together with me. I received 2 nasty gashes on my left leg and bruising. Another long bloody gash on my right leg that I saw the next day. I sent pictures to Costco CS and asked them to fedex me the correct handle or a truck capable to handle the weight they claim their hand truck could carry. I received an apology letter by email asking me for the date of purchase or my member number. They have not contacted me since. I feel that they have no regard for public safety or responsibility to acknowledge a safety problem. The all aluminum frame fully loaded cannot be supported at a crucial point by a hollow abs bracket that should hold the truck in a safe reliable way to avoid an incident like mine. I have pictures of that day before and after the accident. The truck in use and the broken bracket with my injured leg in the background. I contact you because I guess they don’t think I am important enough since I am not a Costco member and I was carrying SAMS water cases :-)

Emergency room Made Mar 22, 2016, sent to the company Mar 30, 2016, published by CPSC Apr 13, 2016
The product, in the report: Cosco 3 in 1 MAX 1000 lb dolly handtruck. Every time I go to upload pictures, the site crashes.
I purchased a Cosco Hybrid 3-Way Convertible Hand Truck from a local store. I was moving a 130 lb small safe with it in “assist” mode. The safe completely collapsed on me in the process, almost immediately. My thumb got caught in something and my knee was whacked hard as it fell. I also ended up falling into a picture frame from my engagement to my wife, which shattered (and is ruined) and left me in the ER getting stitches on my elbow until 3AM on a work night. The rear “assist” section of the dolly is completely bent, which is surprising, given that it is rated for 1000 lbs and I was carrying 10% of that.

No one hurt Made Aug 26, 2015, sent to the company Oct 9, 2015, published by CPSC Oct 26, 2015
The product, in the report: Three in One hand truck
Three in one hybrid convertible hand truck purchased by caller somewhere between August 14th, and 19th, 2015. This hand truck is designed to be used upright, at 45 degrees and flat, however the caller was moving two five gallon bottles of water at a time which were about 45 lbs each sometime a few days after purchase using the hand truck and it collapsed multiple times. He removed one of the 45 lb water bottles and the product flipped to the upright position from the 45 degree position and he states that if he had been in the way he could have been very seriously injured. The caller is reporting that when this hand truck collapses there is a potential for limb loss due to the way the unit collapses. If the users hand or foot are in the way the limb could be injured or lost. There are no injuries being reported in this instance.
The product was returned to the place of purchase on August 25th, 2015. The caller has not contacted the manufacturer and does not intend to contact them.
