My toddler, 19 months was using the Pottery Barn Lamb rocker(https://www.potterybarnkids.com/products/lamb-critter-nursery-rocker/?cm_src=WsiPip1&recstrat=View-View-1%7CVCB-GRP-AFF-QNT). She leaned forward while rocking which caused the entire rocker to tip forward and her to fall forward and off the rocker.
