Death of 19 day old infant while using Graco DuetConnect LX Swing and Bouncer. Infant was placed in bouncer for over night sleep without the head support pillow. Infant’s head believed to be head down position.

The product, in the report: Graco Duo 2 in 1 Swing and Bouncer - Brown/Tan I believe we still have the bouncer portion at a family member’s house.
Our 4 month old daughter was placed into the Graco Duo bouncer seat on the floor for a nap. The seat was not placed in the swing portion, nor was it in the room. She was placed in a velcro swaddle wrap in the seat and the tray placed down, but was not buckled in. Our daughter was found with her face down in the crevice of the seat (where the babies butt would be) not breathing. I wanted make CPSC aware of this because of the recalls of the [REDACTED] and other similiar baby seats.
