The product, in the report: Summer Baby Seat Model 4-in-1 Superseat This is a 4 in 1 seat that can be used as a booster, activity seat, support seat, and feeding seat. It attaches to a chair via straps. The tray turns 360 degrees to change from activity seat with tops attached along with with the attached bar that is dangerous because it cannot be secured to a feeding tray.
Minor incident however it could have been worse. The seat was in the activity seat position with the attached toys and metal bar with beads that move. The bar does not secure into the seat it just sits in the holes so it it easily pulled out by a baby. My daughter had pulled one side out a few times but this time she got the whole bar out and hit herself in the head with it. If the bar had a way to secure it to the tray so that babies could not pull it off and swing it around it would be safe but this is not the case!

The tray was pushed off the booster seat style high chair. The child fell off the high chair after the straps that held him in place failed.
The tray can be pushed off from its locking mechanisms The straps can be loosened by the baby leaning forward and/or pushing with his stomach.
2 mechanisms that we’re designed to keep him safe and secure failed one after the other.
I have only had the high chair for 1 month.

Emergency room Made May 18, 2013, sent to the company May 28, 2013, published by CPSC Jun 18, 2013
The product, in the report: summer infant 3 stage booster seat
4 mo old infant in a summer infant 3 stage booster seat. The infant’s 9 yo sister spun the surrounding tray to entertain. Infants’ left arm was pushed backward because if was wedged against one of the vertical toys on the surrounding tray. This was witnessed by father in room. As sister spun the tray, the toy pushed infants’ arm behind her resulting in pain and decreased movement of the left arm. Pain improved with acetomimophen but progressive discomfort and decrease use of left arm prompted Ped ED visit. Radiographs were negative for fracture. A nursemaid’s elbow reduction procedure was performed with a palpable pop noted. Observation in ED revealed complete resolution of symptoms thus verifying nursemaid’s elbow.

No one hurt Made Oct 18, 2011, sent to the company Oct 26, 2011, published by CPSC Nov 9, 2011
The product, in the report: Summer infant Disney Lil’ Speedster Booster seat The Disney Lil’ Speedster Booster seat by Summer Infant will make your little boy feel like he is sitting in a race car during meal time. This booster is easy to clean, has removable dishwasher-safe tray and has a 3 level height adjustment. Fun cars design will help keep your little boy seated during mealtime Fold down seat back for travel and storage 3-level height adjustment Removable and dishwasher-safe tray 3-point restraint system
The straps that hold the seat to the car can come lose while my child is siting in the chair. The back strap comes loose more than the strap that goes under the seat. This causes the chair to slide forward while my child is sitting in the chair and could cause him to dangle in his seat holding on just by the strap. It says it will hold up to 50 pounds and my son is only 30 pounds and it still comes loose.
