    Incident Description:

I am reporting a serious strangulation hazard involving the Graco Slim Snacker Folding High Chair.

This high chair includes a fabric storage hammock attached to the lower portion of the chair legs. The hammock is secured tightly to the legs with screws and does not detach, slide, or release when weight or pressure is applied.

My infant, who is learning to crawl, approached the high chair out of curiosity because items were stored in the bottom hammock. The height of the storage hammock is positioned at a level where a crawling infant can lift their head and neck over the edge to look inside but too high for a crawling infant to lift their head off of the edge.

When my baby attempted to look into the storage area, his neck became caught on the edge of the hammock fabric. Because the hammock is screwed into place and does not give way under stress, it created a rigid edge that trapped his neck. He was unable to lift himself back up or free himself independently.

He immediately began screaming due to being stuck. I was within arms reach and intervened right away, lifting him free. This situation could have resulted in strangulation if I had not been present.

The design creates a foreseeable entrapment and strangulation hazard for crawling infants. The hammock sits at a height that allows head access but does not allow easy exit, and there is no breakaway or fail-safe mechanism to reduce risk.

High chairs are commonly present in homes with infants who are crawling or learning to crawl. This bottom storage feature presents an unexpected and potentially life-threatening hazard. Products made for babies should not also be a death trap for babies.

I believe this product poses a serious risk of strangulation or neck injury and should be evaluated for compliance with applicable safety standards and potential corrective action.

I was using the Graco Table2Table Premier Fold 7-in-1 High Chair with my 9 month old child. The tray on it locks, but is not secure. We placed my child in the high chair and locked the tray, but with a small bit of pressure, it fell off! The tray flew forward. The tray has a safety feature attached to it - it has a leg barrier in the bottom of it. With the tray flying off, so did the leg barrier, and a piece of my childs security within the chair. The high chair also comes with wheels on two legs, I think for easy moving. However, these wheels have no locking mechanism. My childs security within sometimes sways/rocks in the high chair, and even at this young age, he is able to physically move the high chair because the wheels are so insecure. I cannot leave him even to get more food from across the kitchen for fear that he will rock himself into something, hurting himself. I have not tried the additional features to make this chair grow with baby, but considering the setting for the youngest/smallest children is so fraught with issues, Im scared to continue using it.

The product, in the report: Blossom 6 in 1 Convertible High Chair, Studio, 22.5x41x29 Inch

We purchased a Graco Blossom 6 in 1 Convertible High Chair, Studio, 22.5x41x29 Inch from Amazon. Our child almost fell out of the chair several times, due to the fact that there is no mechanism to prevent them from sliding out of the chair as they are being strapped in. Lively toddlers resist being kept in one position so it makes strapping in that much more difficult. In other high chairs, there is a piece between child’s legs that is attached to the chair that prevents them from sliding out as they are being strapped in. In this case, the leg piece is attached to the tray that slides out and has to be removed when the child is being placed in the chair.

The product, in the report: Graco DuoDiner high chair bought Jan 2018 Model 1965482 Grey with grey and light teal padded seat cover, Heavy grey tray with large V portion between the legs, a beige removable liner on top of tray

I have a 2018 model Graco DuoDiner high chair. My granddaughter of 22 months grabbed the tray (which was fully pushed in as it was not in use) and the whole chair slammed down on her. The high chair has two front wheels that do not lock, and no wheels in the rear, for ease of moving the chair. When she pulled on the tray the heavy chair tipped forward thrusting the wheels to the rear slamming the chair to the ground. The little girl was pinned underneath the whole chair/tray. It slammed her to the ground flat on her back with the tray crushing her across her pelvis area. This is the first time I have had the chair tip, and I was standing right there. There was no unusual activity, the slight tip sent the wheels straight backwards slamming it to the ground. Very BAD and scary.

Hi my name is [REDACTED] and I’m am extremely concerned About the safety of my daughter we have the graco high chair 6 in 1 it’s the one the had a previous recall but that isn’t my concern the straps on her high chair are extremely unsafe they don’t hold her in at all if she leans forward or try’s to get up even with the front table part attached the traps pull completely out from the back and if I hadn’t been by her and I mean close by her she has came close to falling out... I feel like there needs to be a different way on keeping the strap held into the high chair better because they pull right through and it’s not safe I can even walk a few steps away without having to worry if she is going to fall out plzzz look into this if not I wish someone could see what I mean I don’t wanna worry about my daughters safe and I would hate for anyone else to have to either

The product, in the report: Graco Blossom High Chair

I am writing to inform you of an incident that occurred 11/23/2017 while my 18 month old daughter was sitting in a Graco Blossom High Chair (Model 1812897, Made 2013/03/09). She was sitting in the chair, and was strapped in and had the table in place. While I was feeding her, she leaned back and the entire top half of the high chair came off of the base, causing her to fall backwards on the tile floor. Being that this occurred on Thanksgiving, several other family members observed her falling. We checked her for injuries, and it appears that the back of the chair broke her fall, but only barely. We were very lucky that she was not seriously hurt or killed by this incident. It appears that what caused this incident is that the tabs on either side that hold the chair assembly to the base were aligned with the top holes on the stand and became loose. When I looked at the manual for the high chair, no where does it state how to secure the top part of the chair into the base, nor does it call out any warnings related to this item. I have also informed Graco of this incident.

My son keeps getting his fingers stuck in the high chair tray latch/releases. There is a large button on either side of the under side of the high chair tray that when pressed release the tray. But they happen to be right within a baby’s reach. While my son is eating, he hooks his fingers over the edge of the tray sometimes and gets his fingers stuck between the buttons and the housing around the buttons. He’s not able to actually press the buttons, just keeps getting his fingers stuck! He screams every time it happens mostly because he can’t get his finger tips out by himself. It takes an adult to gingerly pry his fingers out.

The product, in the report: This is the Graco simple switch 2-in-1 convertible highchair

On numerous occasion this product has proved to be harmful. If the tray is not in place and the child moves forward, the seat becomes unhooked at the latch and will enable the child to fall forward or backward. I have had to literally, catch my child from falling backward onto our kitchen floor. It is not a matter of inaccurate installation. It is pretty straightforward and does not require much thought behind securing the product. I can record my daughter unhooking the back of the chair if need be.
