The product, in the report: Evenflo 4-in-1 Eat & Grow Convertible High Chair in the color Poppy Floral
We had an infant injury caused by a product. We have been using the Evenflo 4-in-1 Eat & Grow Convertible High Chair for almost a year. Recently, my baby was kicking her feet and the top of the high chair somehow unlatched from the bottom piece and she went flying, while still strapped into the chair, and landed on her face. She was screaming and crying and we contacted a nurse and was advised to take her to the ER to get checked for a concussion. She was finally cleared but has a bruised and swollen cheek, a little bit of a black eye and a scratch on her forehead. We have been using the high chair properly. Even now, we have tested the high chair and when you slightly shake or pull on it, it will sometimes unlatch. This is VERY unsafe. The pieces that are supposed to latch are not mechanical and can easily move and become loose.

First aid Made Sep 25, 2020, sent to the company Oct 27, 2020, published by CPSC Nov 10, 2020
The product, in the report: Evenflow ModTot, Sante Fe SunsetHigh Chair with an adjustable recline seat. The chair is white with a grey seat pad; high chair height can also be adjusted.
The high chair seat was adjusted back. Baby (11 months old) finished eating breakfast. The seat was adjusted back to assist the baby with tilting his drinking cup. When the seat was adjusted forward, the back of his upper arm was pinched leaving a blood blister.
The warnings and instructions states to “Never adjust the seat back while baby is in the high chair” but it doesn’t instruct not to adjust the seat forward. This area doesn’t have a protective barrier to prevent injury.
This is of great concern since we have 6 chairs at our childcare center.

The product, in the report: Evenflo quatore 4 in 1 high chair
I have been using the Evenflo Quatore high chair for nearly a year. This afternoon I noticed a rotten odor coming from the larger detachable tray. I unscrewed the tray to open it up. On the inside I found food bits and water had accumulated and begun to rot. Absolutely disgusting. There are several small holes in the tray top that allow food, water, and beverages to drip in. I called evenflo to inquire if there was a fix, or recall, or substitute tray. The service representative said all their trays have those holes to prevent drowning.
If the holes are a safety feature, then they should drain freely, not into an enclosed space that creates a mold hazard. Or at least make the tray easier to disassemble to clean thoroughly.

Seen by a doctor or nurse Made Feb 6, 2017, sent to the company Apr 5, 2017, published by CPSC Apr 19, 2017
The product, in the report: high chair intended for children up to 70 lbs. Purchase date: Dec 2015
The submitter noticed that food was getting caught in the holes on the tray. The submitter indicated that she had tried cleaning the holes with cleaning products and soaking it but she could not successfully clean the holes. The submitter contacted the firm on January 6, 2017 to inquire about proper cleaning instructions because she noticed gnats around the tray. The submitter was told to use cotton tips to clean the holes but the submitter advised the Rep. (name unknown) that it did not work. The submitter was told that the holes are there as a safety mechanism so liquid could run onto the floor and not stay on the tray. The submitter offered a new tray but the submitter was concerned that this could happen with the new tray. The submitter was also offered a replacement highchair but she states that the model she was being offered was recalled and when she reproached the firm about it they could not explain why she was being offered a recalled model. The submitter was then offered a refund and was told that she would need to dispose of the high chair and provide pictures to the firm of disposal method. The submitter refused the refund.
On January 10, 2017 the submitter’s husband removed the bolts to disarm the tray and found maggots, mold and gnats in the tray. The submitter advised the firm of their findings. The submitter states that the firm has now offered to pick up the high chairs but the she has declined. The submitter indicated that she has hired an attorney.
The submitter indicated that her sons has been feeling ill since March 2016. The submitter believes the tray has been making her sons sick. The submitter indicated that the symptoms include vomiting, diarrhea, and cold symptoms.
The submitter purchased new high chairs and her sons stopped getting sick.
The submitter has twins and received two of these high chairs as a gift in December 2015. The submitter filed a report on the other high chair for the same issue.

The product, in the report: Evenflo Concertible High Chair Black&Fushia made for feeding small child that’s unable to walk
This product is the evenflo Convertible High Chair model name is Marianna these high chairs were recalled in June of 2012, however under a different model number. My five month old [REDACTED] feel out of it yesterday September,3,2015 at approximately 1:30pm after I removed the tray to clean it. She was strapped in. A couple of seconds later she fell out onto the ceramic tiling in the kitchen with the inner black mesh the chair as as the cushion still on her arms. She was rushed to the hospital and has suffered from a contusion on her left eye. My safety concern is why we’re recalled in 2012 just to be made again three years later ? It’s a disgrace

The product, in the report: Neon Green and black high chair. the seat comes apart from the base to create a chair and table.
My 18 month old son was not strapped into his highchair and starting moving around. He pushed on the tray with his feet, it popped out, he flew forward, face first onto our wooden flooring, on top of the tray. He immediately started screaming. Yes, I think he was more scared than anything, but when I got a chance to look at him, his face was bleeding pretty quick. Somehow, something caught his cheek and pinched it so hard it split the skin.

The product, in the report: Lime green convertible high chair. converts from a high chair to a toddler table and chair
I own a convertible high chair by evenflo. My son 10 months old. He was eating breakfast and playing in his high chair. He leaned forward against the tray, the tray snaped off and my son landed in the floor. He now has a bruise across his nose as well as a black eye and a busted lip. He is pretty terrified to sit in any high chair now after the incident.

The product, in the report: Evenflo Majestic High chair. It has a green and tan plaid cover and the tan frame. It also came with a battery powered toy that snaps into the tray.
A screw fell out of the backrest and caused the back to collapse while my child was seated in it. It is also extremely difficult to secure the tray to the chair frame. I was standing next to my child when it happened, so I was able to catch her. She did not sustain any injuries. This incident happened with my first daughter. I have stored the chair since then and just recently retrieved it for my newest addition. This is when I remembered that there was a safety problem with the chair.

Hurt, no treatment Made Jun 19, 2012, sent to the company Jun 27, 2012, published by CPSC Jul 12, 2012
The product, in the report: Evenflo Convertible high chair
We had just bought his high chair, brand new in box, online, and used it 1 time. The 2nd time I went to put my baby in the chair, and the straps were way too short for my child, who was only about 14 months at the time, so I decided to put the tray on in the meantime, so I could go around to the back to see about adjusting the straps. To my HORROR, my son barely pushed on the tray and it popped off, and he tumbles to the floor. He cried, and was shaken up. I immediatly contacted the online retailer, and I have proof of the date of the message sent, so this is not to “jump on the wagon” with all the other re-calls..I was one of the 1st ones to report this. It was really a trying time, having to sit right next to my baby, taking the high chair down from the higher height, down to the lower level, close to the floor, and I had to sit right next to him, so if the tray did pop off, I was right there. My son still is learning to walk, so I figured that once he can walk himself, then I wouldnt need the tray anyways. I am very happy that its been re-called, and that my claims are valid!

The product, in the report: Convertible high chair Dottie Lime model number
I purchased an Evenflo convertible highchair a couple months ago for my 2 1/2 year old daughter from Wal-mart. One day she was trying to climb into the highchair by herself. She put her foot on the table part and pulled herself up on to the table completely. Once she had both feet on the table she was using the tray to hoist herself into her seat and even though the tray was in correctly it completely came out of place and my daughter tumbled off and broke her right clavicle.

The product, in the report: Evenflo Convertible High Chair- Dottie Lime color
I have 2 of the Evenflo convertible high chairs and withing 1 week of setting them up, the tray started detaching from the chair and twice my son fell from the high chair ( a drop of about 3 1/2-4 feet). He had some bumps and bruises, but nothing super serious (although I think we were just lucky) since then I have been using them on the floor without the table. The trays still come off occasionally but since they are only about 8-10 inches off the floor it isn’t a long way to fall.
