A 3-month old female infant was placed into a baby swing chair (EOM INNOVATION INC.; Batch no.: CR018B20240326; Address: 161-15 Rockaway Blvd. Ste. 210 Jamaica, NY 11434) after having a bottle of baby formula, and was found about an hour later to be not breathing and unresponsive. She was later declared deceased.
