Made Nov 2, 2011, sent to the company Nov 10, 2011, published by CPSC Dec 5, 2011

The product, in the report: 12” baby bear with 4 pockets - each with an interactive toy. Squeaky fish, chirping bird, rattling squirrel and a teething cracker. String attaches teething cracker to bear.

My 4 month old baby was playing with the Woodours Baby Activity Rattle and Teether Bear (purchased at the baby store “Giggle”)in his crib while under my supervision. A teether is attached to the bear with a string. My baby started screaming, and I found the teether underneath his head, the string tight against his neck deep under his chin, and the plush bear on his other side. The string was wrapped around the front of his neck. I didn’t see the string at first because it was buried deep in his neck. I tried to move the bear and saw that the string was choking my son. As soon as removed the toy, he stopped screaming and seemed fine.

The string is a hazard and should not be attached (sewn) on a toy that is sold as appropriate for newborns and older.
