The reports, as CPSC published them
Reports people made to the U.S. Consumer Product Safety Commission. CPSC publishes each one after sending it to the company named, and doesn’t guarantee they’re accurate. A report isn’t a recall.
Each report is the words of the person who made it, as CPSC published them. The company’s comments, where CPSC published any, are set apart as the company’s own.

The product, in the report: Ocean Tummy Time Triangle- Tri fold baby soft toy with tags and other sensory items. Color and black and white sides
I am reporting this serious safety incident to the CPSC and Melissa & Doug involving a toy used by my 7-month-old son.
Without our knowledge, my son was able to completely detach the green fabric tag from the toy during normal use. We did not see him remove it. We became concerned when we heard coughing and checked on him, nothing was visible in his mouth and he appeared okay.
Over the following several days, he developed a fever, cough, and significant irritability. Today, he passed the detached tag in his stool, and I had to remove it from his anus.
I am extremely concerned and disappointed that a 7-month-old infant was able to completely remove a component from this toy through ordinary use. Infant products must be designed to withstand normal pulling, handling, and mouthing. This represents a serious safety concern and warrants investigation.
