  1. No one hurt

Made Oct 15, 2026, sent to the company Oct 21, 2026, published by CPSC Nov 4, 2026

The product, in the report: “Tummy Time Mirror Toys, Baby Mirror Tummy Time Play Set for 0-6 Months” is the name of the product. The description below says “Large Baby-Safe Mirror, Adjustable Infant Mirror Toy with Black and White High Contrast Cards for Newborn Visual Stimulation” It’s a wood-framed mirror (12”L x 10”W) used for infants to practice tummy time.

I have a tummy time mirror my 11 month old enjoys that I got when she was a newborn. It’s a mirror that you can turn over to use as an easel of sorts to put black and white vision cards. It stands up in a triangle shape and has hinges so you can change the degree the mirror is sitting at. The other day we were in the play pen together and I noticed that two metal screws had fallen out of the hinge of the mirror! Thankfully I was there with her and noticed before she saw them and put them in her mouth, but this is a dangerous choking hazard! I went to leave a one-star review on Amazon and read SIX other reviews that said the exact same thing happened to them. This product is marketed toward babies and needs to be recalled before someone’s baby gets seriously hurt.
