I received the product, “The Ultimate Crib Sheet” made by Summer Infant, Inc. The compny address per the label was Rhode Island 02895; item #92470; UPC #012914924709. Upon the second washing of this product in the washing machine (per the directions wash in cold water, tumble dry low), my husband noted that several of the snap buttons came off in the washing machine. Should this have occurred during use, this could have been a choking hazard.
