Illinois Child Death Review Team (https://www2.illinois.gov/dcfs/aboutus/policy/Pages/com_communications_CDRT.aspx) was reviewing a death of a 3 month old male who died while sleeping in a Rock ‘N Play. He was sleeping on his back, with the padded insert that comes with the product and was found unresponsive by his parents.
