No one hurt Made May 30, 2021, sent to the company Jun 2, 2021, published by CPSC Jun 16, 2021
The product, in the report: MAYFAIR 888SLOW 000 NextStep2 Toilet Seat with Built-In Potty Training Seat, Slow-Close, Removable that will Never Loosen, ROUND, White Model 88SLOW Next Step 2 Child Training and Adult Seat
We have a toilet set with a built-in child’s seat that is held in place with a magnet. Today my 3 year old came to me holding a small (about the size of a dime) very powerful magnet disk that had dropped out of the toilet seat. I had not noticed any wear and tear on it prior, and it is not heavily used. It appears the magnet just fell out on its own. I am so thankful she chose to bring it to me instead of swallowing it, as we would have had a very different end to this story. The toilet seat was purchased from Amazon on July 3, 2020: MAYFAIR 888SLOW 000 NextStep2 Toilet Seat with Built-In Potty Training Seat, Slow-Close, Removable that will Never Loosen, ROUND, White https://www.amazon.com/dp/B07ZQT2R9N/ref=cm_sw_r_cp_apip_apB1GN33qbz8J

Emergency room Made May 4, 2012, sent to the company May 11, 2012, published by CPSC May 25, 2012
The product, in the report: Child toilet seat
My 3 and-a-half-year-old son was sitting on a child’s toilet seat when he fell forward to the floor. In the process of falling, the base of his penis got caught on a sharp edge on the inside lip of the child seat resulting in a severe cut requiring 7 stitches.
This sharp edge appears to be the result of the plastic molding process where the mold comes together.
