  1. No one hurt

Made Sep 22, 2022, sent to the company Sep 27, 2022, published by CPSC Oct 12, 2022

The product, in the report: Product purchased 2017. Skip Hop Moonlight and Melodies Nightlight and soother. It is an owl that lights up plays sound and projects stars. Adapter is a white 5v two prong to USB. Adapter model number ZAU-F050055A-01 ShenZhen Zhongling Electronics Technology Co. Ltd. It is UL listed E479473. The adapter is the part that broke apart.

My 3 year old bumped the adapter plug where it was plugged into the wall. The prongs and a small plastic plate snapped off in the wall receptacle. This left the back end of the energized prongs exposed (still plugged in). I stopped him before he touched it so he was not injured. However he could have been seriously burned or shocked. The flat side of the adapter with the prongs in it was clearly a separate piece glued into the body of the adapter. The glue apparently failed under very little force- just a bump from a toddler’s arm while plugged in.

Made Aug 27, 2018, sent to the company Sep 5, 2018, published by CPSC Sep 26, 2018

The product, in the report: The item is the Skip Hop Moonlight & Melodies Nightlight Soother Elephant

The Elephant Nightlight Soother was being used in my sons room every night since he was born Feb 2015. Its been great and we love it. Sadly today the cord broke where it connects to the USB which then connects to the plug-in. We rarely move the elephant so this came as a shock when we unplugged it to clean his room. I was looking online to see if we could simply get a replacement cord when I came upon a recall notice for the same product.

The product, in the report: Skip Hop Owl Nightlight Soother

This nightlight soother was plugged in and I was standing right beside it when the plug suddeny started sparking and making a very loud popping noise. We were using a power strip and the only damage to the power strip was the outlet that this item was plugged in to.
