The product, in the report: portable infant crib with removable changing table
On the date of the incident an infant child had her two lower teeth damaged from a crib utilized at the school in the infant room. While a teacher was retrieving the infant from the crib it was not realized the infant had clamped her teeth on the bracket on the edge of the crib that is used to mount an external changing table. Subsequently when the child was picked up her teeth were pulled out and down. The family was notified immediately and CPS was notified by self reporting procedures. The crib was approved for use by licensing prior to installation in the infant room. The components responsible for the incident were removed to make the crib in line with other cribs without this feature. In addition a safety stand down and changes to current safety procedures are being revised to include process improvement and composite risk assessment. The incident is still pending investigation and the teacher is on administrative leave until further notice. An investigation by the school is currently under way and it is being checked for recall of this item or any safety notifications. All manufacturer’s warnings were followed and there was no instructions dealing with these components.
