No one hurt Made May 9, 2015, sent to the company Jun 10, 2015, published by CPSC Jul 1, 2015
The product, in the report: WubbaNub “Little Lamb” pacifier. Model number: W32351
My infant was using a “little lamb” WubbaNub pacifier by Mary Meyer Corp in his carseat. When we reached our destination, I noticed small hard plastic beads on his chest and in his neck folds. The seam of one of the lamb’s legs had come unsewn, revealing the plastic beads within. It is unclear whether he swallowed any; however, this is a serious potential choking hazard. He is five months old, does not have any teeth, and was using the toy appropriately.

No one hurt Made Apr 15, 2014, sent to the company May 1, 2014, published by CPSC May 15, 2014
The product, in the report: Wubbanub brand pacifier. This particular one has a brown and beige monkey attached to a teal pacifier.
Baby was being rocked to sleep while sucking on a Monkey Wubbanub pacifier toy. It is a pacifier attached to a stuffed monkey. Baby started sucking on the monkey’s leg. The baby started flailing around. He was not making any sounds through his mouth. Looked down to see that the baby had shoved the leg completely in his mouth and could not breathe through his mouth. Toy was removed and baby was ok.

No incident, no one hurt Made Dec 19, 2012, sent to the company Jan 8, 2013, published by CPSC Jan 30, 2013
The product, in the report: Wubbanub: Pacifier with small stuffed animal attached
None at this time, but I believe it to be a matter of time. There is a small stuffed animal attached to a pacifier that is close to baby’s mouth and nose. It could easily during sleep come in contact with either location and cause the baby to suffocate. With all the information about SIDS and removing extra blanket, clothes, bibs and stuffed animals, I for the life of me can’t figure out how this is a safe product. Would like to see it recalled before a little baby rolls over on it and can’t breath or the animal comes in contact with their nose or mouth during sleep.

No incident, no one hurt Made Jan 17, 2012, sent to the company Jan 25, 2012, published by CPSC Feb 8, 2012
The product, in the report: An infant pacifier that has a stuffed animal permantely attached to the end of the pacifier. This product is suggested to be used for ages infant to 6 months. The stuffed animal is about 6 inches in length.
The report I am about the detail is not from an actual instance, but can help prevent one that has a high probability of happening with this product. Please copy and paste link in your internet browser:

The above product is suggested for infants to 6 months. As a Sudden Infant Death Syndrome (SIDS) trainer for my state agency, I would hypothesize this product would constitute a risk to children. “Stuffed animals” in an infant sleeping area or crib is recognized as a SIDS risk due to the chance of suffocation and reduction of air flow around the sleeping area. This product would possibly allow the stuffed animal product to be next to an infant’s breathing area. Even by the products own admission on their website, “Carefully calculated weight and size of plush animal keeps the WubbaNub™ near baby’s mouth, whether lying on side or back, the car seat, sling or stroller. In use of any infant product, safety is a # 1 concern and this product does not ensure this safety. It may also present a false sense of security to parents based on the company’s claims.
I hope there has not been on incident with use of the product. In my opinion, it is only a matter of time if this product gains popularity or use. This item was brought to our attention through a State Licensed Day Care Center parent who insisted her child use it in a center setting. We stated we would advise against it since stuffed animals and other soft bedding are not allowed in infant sleeping areas.
Thank you so much for your time.
Sincerely,
[redacted] State Child Care Licensing Trainer - Houston, Tx

No one hurt Made Nov 1, 2011, sent to the company Nov 9, 2011, published by CPSC Dec 2, 2011
The product, in the report: Wubbanub: a dog with a pink belly that has brown polka dots and a pink face. The facial features are made of brown embroidery thread. The dog has a pacifier sewn into its mouth
UDATE FROM THE CONSUMER: The dog is actually pink on the top and brown with pink polka dots on the bottom.
My 9 month old daughter was given a brown and pink wubbanub dog when she was born. Recently, the embroidery that is the eyes and nose on the dog began to unravel and leave a string long enough to get caught around a finger or toe. I cut the strings and haven’t had any issues since. She was not injured by the toy/pacifier
