Hurt, no treatment Made Aug 17, 2026, sent to the company Aug 20, 2026, published by CPSC Sep 4, 2026
The product, in the report: Tommee Tippee Breast-Like Night Pacifier, 6-18 Months, Breast-Like Baglet, BPA-Free Binkies, Glow in The Dark Technology, Yellow/Green, Pack of 4
On August 16, 2026, my 11-month-old son was using a Tommee Tippee pacifier normally during nap time when his finger became fully trapped inside an opening of the pacifier. His finger was unable to come back out, and the pacifier was effectively stuck around his finger. His finger began turning purple while it was trapped, and he was screaming and crying in distress. The discoloration was concerning for compromised circulation. I had to intervene and remove the pacifier from his finger. This was not the result of the pacifier being modified or used improperly. The pacifier was being used as intended by an infant. The design allowed an infant’s finger to enter an opening far enough that it became trapped and could not be easily removed. I am reporting this because I believe the design presents a serious finger-entrapment and circulation hazard for infants and could result in significant injury if a caregiver does not immediately notice the problem. An infant may be unable to communicate what is happening or remove the pacifier independently. I am requesting that CPSC review this product and determine whether the design presents a safety hazard to infants. I have retained the pacifier and can provide photographs and additional information regarding the product.

Emergency room Made Jul 28, 2026, sent to the company Jul 31, 2026, published by CPSC Aug 14, 2026
The product, in the report: Tommee Tippee Ultra-Light Stay-Put Pacifiers
Tommee Tippee Ultra-Light Stay-Put Pacifiers
We have used these pacifiers for months and have never had an issue until yesterday.
Our son, [REDACTED], got his thumb stuck through one of the openings in the pacifier. We believe it was only stuck for a couple of minutes before we noticed he was becoming very fussy. Once we realized what had happened, we were able to remove the pacifier from his thumb. At the time, I didnt think much of it, and we continued on with our day.
A couple of hours later, I noticed that his thumb was becoming extremely swollen and that a large blister was developing on the top of it. His thumb became so swollen that the skin was extremely tight, and I became concerned about the possibility of him losing circulation. We decided to take him to urgent care to have it looked at.
The doctor told us there wasnt much they could do other than have us monitor it very closely and watch for any signs of decreased blood flow to his thumb.
Thankfully, we are keeping a close eye on it, but I wanted to share our experience because I cant imagine what could have happened if this had occurred overnight or during a long nap and we hadnt noticed right away.
I am not posting this to say that every child will have this happen, but I do want to bring awareness to the possibility. Please check your childs pacifier and be aware of the openings, especially if your child puts their fingers through them.
Tommee Tippee we wanted to share this experience and bring it to your attention. We would appreciate any information you can provide regarding this type of incident and whether there are any recommendations for preventing it from happening.

No one hurt Made Apr 10, 2026, sent to the company Apr 14, 2026, published by CPSC Apr 28, 2026
The product, in the report: Tommee Tippee ultra light stay put silicone pacifier
My daughter was tossing and turning in her crib for a good 30 min and woke up twice, said ouch and went back to sleep. The third time she did that I went in to check on her thinking she was getting her molars and her gums hurt. I probably asked her over ten times does your mouth hurt, where does it hurt Show mommy, but she was half asleep and didn’t respond. I again chalked it up to it being gum pain, and went to grab the kids ibuprofen from her closet. When I laid her down on the changing table to dose the meds, she said ouch again and lifted her hand. I had my phone flashlight on, so I was finally able to see that the binky was very tightly stuck at the base of her pointer finger, cutting off circulation to her poor tiny little finger and it was completely white and swollen. My heart dropped and I tried to gently take it off, but she started screaming like I was amputating her whole arm, so I quickly yanked it off. Poor girl was hysterical. After a minute she calmed down and I rocked her back to sleep.

Seen by a doctor or nurse Made Dec 9, 2025, sent to the company Dec 11, 2025, published by CPSC Dec 30, 2025
The product, in the report: Tommee tippee breast like pacifier 0-6 months BPA free pack of 4 from Amazon.
My baby was using the Tommee Tippee breast-like pacifier during a trip to the mall. As I was pushing the stroller I stopped to talk to him, I noticed he was holding his pacifier and chewing it. He was comfortable and I didnt think much of it, but then I noticed something white inside the pacifier. As I looked closer it was his finger completely inside the hole on the back of the pacifier. As I tried to pull it out the finger was stuck as if it was a vacuum. He got a little fussy as I wiggled it off. His finger was completely white from his pointer finger tip to the knuckle. And he had 2 indentations around the knuckle as if he rubber bands cutting off circulation. I massages his finger and blood circulation returned. However his finger was dark pink reddish color for about 15 min. He was able to move it. I am a nurse practitioner and have training on what to look out for, so I observed him. We have a doctors appt tomorrow and will be discussing with his doctor for any possible nerve damage and how to know. The product is aged for 0-6 months. He just turned 6 months last week. If I had not caught it on time he may have gotten tissue necrosis. This could also happen at night, as many mothers use pacifiers for the babies to sleep. This design is completely unsafe and Im not the first mom that has experienced this. Just not well reported or no awareness spread yet.

What happened that made you want to report? 3 months old baby swallowed a Tommee Tippee Ultra-Light Silicone Pacifier (0-6 months) , the product was purchased through Amazon.com The pacifier has been advertised as 0-6 months old but the size is too small of that big range of age as for babies that have a hight percentile grow curve when born. My 3 month old son , was taking a nap when the pacifier was laying on his mouth , this is a silicon based pacifier single piece, seemed to got into his mouth after a few minutes in the wrong angle, while I , the father was in the same room , I step out 30 feet away for 2 minutes when I heard him highly alarmed and crying loud, my response time to his location was about 3 -4 seconds to aid him, at first , I was not aware of a pacifier in the month until I did not see it nearby and flip him up to get a view of his mouth, the pacifier had allocated near the right cheek , his constant motion was making it very hard to get it out and the saliva made the silicone of the item very slippery impossible to reach, calling emergency response after trying for 3 minutes and unfortunately they could not remove the items either, I just was getting deeper, so we took him to ER , where the pacifier was safely remove but leaving a laceration at the top of his mouth ceiling . 5 days of hospital later he was discharged after being able to feed properly.

Caused by the device allocation within the mouth , does not have a secure way to extract , the product is very soft which makes it hard to get from a 3 month old mouth , also becomes slippery when saliva wraps the pacifier or made contact with the item silicon.

The product, in the report: Tommee Tippee Ultra-Light Silicone Pacifier, Symmetrical One-Piece Design, BPA-Free Silicone
Incident happened in the care of Daycare: Daycare personnel laid my 8 month old daughter down to nap with her pacifier in her mouth as she normally does. Then she realized that my daughter was choking and saw that the whole pacifier was inside of her mouth. She attempted to remove the pacifier with her finger without success as the pacifier was pulled down her throat from the crying and sucking. She immediately called 911 and they took my daughter to the emergency room where they sedated her and was able to remove the whole pacifier from her throat (photo attached). Luckily, the pacifier was lodged in her throat in such a way that it wasn’t fully blocking her airway. She suffered throat trauma, lacerations, bruises and emotional trauma. She was hospitalized for almost two days.

The product, in the report: Tommee Tippee brand pacifier “Fun styles” line (18-36 months)
My son has been using Tommee Tippee pacifiers since he was born. Once he turned 6 months old, he strictly used them for sleep. They never leave his crib. I clean them thoroughly every 2-3 days and replace them frequently. When I got my son from his crib in the morning, in his hand was one of his pacifiers that was severely damaged. There were no definite bite marks, as the nipple seemed to have just torn away from the shield. It was honestly hanging on by a thread. I immediately called Tommee Tippee. The company wouldn’t do anything about it because on the fine print in the pacifier packaging, it says to replace them every 3-4 weeks. No nipple of a pacifier should separate from the shield. This easily could have choked my son.

The product, in the report: Tommee Tippee pacifiers 0-6 months “R”Web#: 059316 SKU: 8DBB439E UPC/EAN/ISBN: 666519330665
Mold growing in and around the outer portion of a Tommee Tippee pacifier, on numerous pacifiers. The company lists on its wensite as a suggestion to toss pacifiers afyer a month of use and on a pamphlet on the inside of the pacifiers but who would think to read this on pacifiers. With that said the design of these pacifiers and te way they trap water even before a month there is no guarantee it won’t create mld even with the safest of cleaning. I contacted the company and they are taking zero responsibility for this design flaw. I tead a review on Target.com of someone who also had this problem. This is the only pacifier our baby will use and now I am highly concerned after finding mold on several pacifiers and after contacting the company getting zero answers for the design flaw. They seem to think because they suggest tossing the pacifiers after a month or at a sign of wear and tear its ok their pacifiers grow mold? What baby is putting that much wrar & tear on pacifier that you would say oh this needs to be tossed? Not my baby. I clean these religiously and air dry them and they still got mold, this is a huge problem.

We were eating dinner and my husband was holding our 7 mo baby on his lap. We gave him a Tommee Tippee pacifier and then noticed a couple minutes later that he was choking and had stuck the whole pacifier in his mouth.

The product, in the report: Tommee tippee Closer to nature clear pink silicone pacifier 3+ month x’s2
As usual, I inspected the tommee tippee pacifier that I have been giving my daughter, who is 3 months old. We purchased the pacifier approximately 3 weeks prior to the incident. I gave it to her before her nap and laid her down. About 5 minutes after doing so she began to cry. I went to check on her and realized that she no longer had her pacifier, this is a common reason for her to fuss after being put down for a nap. I find it and it pokes me. As I examine it, part of the plastic ring on the outside of the pacifier had broken off. I immediately panicked in fear that the missing piece may be in her mouth. Luckily, it was by her head and she didn’t choke or swallow the piece.

The product, in the report: Tommee Tippee BPA free 2-pk clear blue pacifiers 6-18 months product link: http://www.toysrus.com/product/index.jsp?productId=11005543
I bought a 2-pack of Tommee Tippee BPA free clear blue pacifiers 6-18 months from my local Toys R Us store. Product link: http://www.toysrus.com/product/index.jsp?productId=11005543. We had them for about 2 months when one day my son was sleeping in his crib and I noticed his pacifier next to him. The pacifier was broken. A part of the hard plastic on one side was missing, later to be found in his crib also. I do not know if my son broke it off by teething on the side of the pacifier or if he stood on it in the crib causing it to break. Regardless, the 1/2” piece of hard, sharp plastic that broke off could have been a HUGE choking hazard.
