Reports people made to the U.S. Consumer Product Safety Commission. CPSC publishes each one after sending it to the company named, and doesn’t guarantee they’re accurate. A report isn’t a recall.

Each report is the words of the person who made it, as CPSC published them. The company’s comments, where CPSC published any, are set apart as the company’s own.

Made Feb 4, 2026, sent to the company Feb 9, 2026, published by CPSC Feb 24, 2026

The product, in the report: The product is a Philips Avent Soothie Orthodontic Baby Pacifier, Model SCF192/70. It is a one-piece, 100% silicone, BPA-free pacifier intended for babies aged 3 to 18 months. The product is designed for infant oral soothing and was sold as part of a four-pack in assorted colors.

An infant pacifier manufactured by Philips (Philips Avent) experienced a component detachment during normal use, resulting in ingestion of the detached piece by an infant. The product was being used as intended and under adult supervision at the time of the incident. Following the incident, the infant experienced approximately one week of distress, including persistent crying and visible discomfort. The detachment presented a choking and ingestion hazard. The product has been retained and preserved by the consumer, and photographic documentation is available. The manufacturer was contacted, including an executive-level escalation, but no response was received.

The product, in the report: Philips Avent Ultra Air Pacifier - 4 x Light, Breathable Glow-in-The-Dark Baby Pacifiers for Babies Aged 6-18 Months, BPA Free with Sterilizer Carry Case, SCF376/08

My youngest is 6 months and one week old. He’s using Philips Avent Ultra Air Glow in the Dark pacifiers in size 6-18 months. He’s definitely under the 50th percentile, so this would not be a case of him being too physically large for this pacifier. On Saturday, March 22, I was carrying him and he was using his pacifier, sucking on it and practicing putting it in and out of his mouth. He screamed a horrible scream, and I looked down to see his hand over his mouth --i moved it aside and found the pacifier was stuck inside backwards (ring and decorative part inward, nipple outward). It was wedged behind his gum, like behind where his teeth would be if he had them. I had to work at it to get out out and he was inconsolable for some time after. No visible damage, but you could tell from the screams that he was in pain.

The product, in the report: Philips Avent 2pk Ultra Soft Pacifier 0-6 Months

4.5 month old had Philips Avent Ultra Soft Pacifier 0-6 Months stuck in his mouth. While I left My baby in a safe place with the pacifier properly in his mouth, and stepped away, I came Running back about three minutes later to his screaming. He had the pacifier turned sideways, rotated 90 degrees to the right, and wedged in his mouth where he couldn’t pull it out. While panicking he continued to bite it, even though his mouth was gaping around it. I attempted To pull it straight out, and it wouldn’t budge. I had to turn it 90 degrees clockwise while wedged in his mouth which there wasn’t really enough room to do. His gums where the pacifier edge was making contact were red and puffy minutes after the pacifier was removed.

The pacifier then flipped the “right way” so the pacifier back was flat against the inside of her mouth. She began gagging and clawing at her face to attempt to remove the pacifier- she could not and I prayed it out of her mouth.

The product, in the report: Philips Avent pacifiers

On Thursday May 24 I made a purchase at my local target store, I purchased two packages of the Philips Avent pacifier for my newborn son. Today on Friday May 25 I went to wash the pacifier and noticed the silicone part was filled with water, I assumed this was a product defect and it immediately. Again, today on May 25th I wash a new pacifier under soapy water and the silicone part is filled with water, I discarded an additional pacifier and assume it’s best to sterilize these pacifier in my steam sterilizer, to my dismay I find two brand new pacifier with moisture in the silicone part (again is after being run in the sterilizer). I looked at the companies web site for support and find that they suggest to “squeeze the water out”. As a consumer and parent I don’t find this acceptable, it’s clearly a health concern because these products are holding moisture which is a breeding ground for bacteria, I’m glad I noticed this before my son could have become ill. I’ve also taken pictures

The other pacifier from the set was also discarded as the same kind of tear was discovered while washing it and seeing the nipple fill with water.

My five month old son was at day care with his newer Philips Avent pacifier for 6 to 18 months, the night one with the glow in the dark handle. he had been using the pacifier earlier in the afternoon. when he was sitting at the high chair table and the pacifier was on the table they noticed the nipple was partially detached from the holder. Both the day care staff and myself are alarmed that this is even possible. My son has no teeth and there is no way that this should have happen. the pacifier is obviously defective and could have become lodged in his throat and god forbid he was unable to get care in time he could have died. I have bought several of these pacifier and have since this incident collected them all and placed them out of reach from my son until I can find out from the company the problem with this product.

Last Friday on 9/16/11, My 4 month old daughter was playing on the floor. She had a rattle and her pacifier which she likes to play with also by pulling it out of her mouth and putting it back in. She has chewed on the sides of of the pacifier before and would put the pacifier in sideways but just enough to chew on the end of the mouth piece, the nipple and mouth piece are suppose to be not to able to allow it to go any further than that. I was on the couch and got up to change her diaper, she wasn’t making any noise so I looked up at her as I often do anyways. When I looked up i saw my child’s face bright red and the pacifier completely wedged/jammed into her mouth. There was not one part of this pacifier even remotely close to being out of her mouth, the whole entire thing completely wedged in there!! I panicked and attempted to remove the pacifier, to my shock it didn’t budge. I then had to rip the pacifier with a good amount of force to get get it out of her mouth. She made no noise for about 8 seconds and began screaming crying hysterically. I held her close and wanted to see if her mouth was okay. The roof of her mouth was bright red, scratched, and swollen. My Daughter was not breathing while the pacifier was jammed in her mouth, she would have died had I not been there. She was using this pacifier the same way every other child does, teaching herself how to put it in and out of her mouth, sucking on it, nothing out of the ordinary. She should not have been able to do what she did.
