The reports, as CPSC published them

Reports people made to the U.S. Consumer Product Safety Commission. CPSC publishes each one after sending it to the company named, and doesn’t guarantee they’re accurate. A report isn’t a recall.

Each report is the words of the person who made it, as CPSC published them. The company’s comments, where CPSC published any, are set apart as the company’s own.

The product, in the report: NUK Comfy 100% Silicone Pacifier. Model # 2156581. This pacifier has a hole in the front on the bottom where my son was able to put his finger but couldn’t get out.

My son (less than 2 years old) fell asleep with the NUK Comfy Silicone Pacifier. During the night, his finger became lodged in the front hole of the pacifier. We monitor his room with a camera, but he didn’t make any noise or movement to indicate he needed help so unfortunately, I only discovered the issue in the morning when I found his finger completely black and blue, still stuck in the pacifier, clearly suffering from restricted circulation for several hours. After a visit to the emergency room, we’re thankful there was no permanent damage, but this could have ended much worse. I’ve already reached out to NUK to report the incident and urged them to recall this particular pacifier until they can redesign it to prevent such incidents from happening again.

The product, in the report: Green silicone pacifier

My two month old was using a Nuk silicone pacifier. I happened to be holding it and pushed on the side, and realized the pacifier was torn. It was barely obvious to the naked eye unless manipulated. The pacifier has been sitting in a pacifier holder, unused and has not been subject to any kind of force or anything to cause it to break. Had it torn all the way, he would have choked.

The product, in the report: Nuk pacifier, size 6-18 months, blue shield with clear nipple and glow in the dark handle. Model 69739

While on my morning commute with my 7 month old son, I suddenly heard him screaming in pain and fear from his carseat in the back of the car. After pulling over on the side of the road I discovered he had shoved his Nuk pacifier backward into his mouth and lodged the handle vertically in his mouth holding it open but he couldn’t spit it out. Thankfully the shield kept it from going in any further so he couldn’t swallow it but if he was just a little bigger he could have choked and not been able to cry out for me to save him. He was scared and has some bruising on the roof of his mouth but he very easily could have gotten cut or died had the circumstances been different.

The product, in the report: Nuk orthodontic pacifier 0-6m, white/clear, came in the Nuk Newborn Gift Set of Simply Natural bottles.

My 5 month old daughter was using the Nuk newborn orthodontic pacifier with handle (0-6m) that comes in the Nuk Newborn Simply Natural bottle gift set. She put the pacifier in backwards and the handle became stuck in her mouth. It was really stuck in there, but I was eventually able to remove it by twisting it. It obviously hurt and scared her and she cried for a while after its removal. The handle was lodged upright, prevented her from closing her mouth and scraped against the roof of her mouth.

The product, in the report: NUK Orthodontic Pacifier, 6-18m

Over the past two weeks, we have had at least 5 NUK pacifiers tear right along the NUK name that is written on the pacifiers. Our most recent event was when the tip of the pacifier completely tore off and our daughter had it in her mouth. Thankfully, she did not choke on the piece because we caught it in time. We replace our pacifiers every few months and own at least 20 pacifiers at any given time for the crib, day care, car seats and for use around the home.

My daughter is 5 months and was trying to put her binky in her mouth, she got the hard plastic part in her mouth and it got stuck. I tried to turn it and it was completely jammed and her jaw locked. When I finally got it out it cut her gums up. The brand is NUK and it has hard plastic on the outside with a silicone nipple.

The product, in the report: Small pacifier

Yesterday my baby was playing with a new pacifier it is a nuk pacifier so I turn my head for less than a second when I turn back around the baby had the part of The Pacifier that you can grab onto that has a slight handle part on it and has mouth and it was wedged longways oblong into his mouth upon pulling it out and cut his gum open and he bled all over his bib it wasn’t like heavy blood or anything I was like light blood mixed with spit but I do have pictures I think that should be raised as a sure risk for other babies and this report should be looked into this pacifiers extremely small so considering how small it is maybe that should be considered when people are making pacifiers I would have never known this honestly and I feel really bad I even gave it to him he had a sore on his mouth and it hurt him to suck on a bottle or anything yesterday thank God today he’s doing better

Made Nov 3, 2016, sent to the company Dec 5, 2016, published by CPSC Dec 27, 2016

The product, in the report: Pacifiers

Purchased from two different retailers but doesn’t remember when.

This caller is reporting that his one year old son has a pacifier and that when he took from his child saw that there was actually only a thin line of rubber between the nipple and the part that keeps the nipple from coming loose and potentially causing the child using the pacifier from choking/suffocating on that portion of the pacifier.

He contacted the manufacturer who told him that his son’s teeth were causing the pacifier to rip apart.

He purchased another pacifier and the same incident occurred in a very short period of time.

He then checked the four other pacifiers that had not been used yet as of that date and saw that there were small pin holes all around the areas between the nipple and the larger part that stops at the mouth to keep the pacifier nipple from detaching into the child’s mouth.

He then contacted the manufacturer again who again stated that the child’s teeth are tearing the pacifier apart.

This caller states that babies get their teeth always when using a pacifier and they shouldn’t be coming apart.

He is reporting this as a choking/suffocation hazard

There are no injuries in this case and he contributes this to his being there before the nipple came apart from the pacifier.

He has contacted the manufacturer on multiple occasions.

He said that there should at the very least be a warning that parents should check the pacifiers daily for any safety hazards.

The caller states that the manufacturer advised him that they were sending someone to pick the pacifiers up but he declined stating that he was saving them for the CPSC Investigation Team.

The product, in the report: NUK pacifier, latex

My 6 month old son was on the floor in our living room when I look over and saw that his pacifier chain had come lose. On second inspection I realized that the NUK pacifier that was attached to the chain had broken into 3 pieces. I couldn’t find the third piece and then realized that my son was still sucking on it. The nipple part was still in his mouth, and I shudder to think what could have happened.

I reached out to NUK directly and they arranged for someone to pick up the pacifier so that they can assess what went wrong.

I’ve been using these pacifiers for years with both of my children, and I was utterly shocked when I saw what happened. This was a new pacifier; we’ve had it for just a month. There is no way my son could (or should be able to) tear apart a fully functioning pacifier just by sucking on it, whether it’s hooked to a pacifier chain or not.

The product, in the report: Nuk Design OrthoStar Pacifier

No injuries were sustained, but the Nuk Design OrthoStar pacifier poses a huge choking hazard. TWO of my child’s OrthoStar pacifiers have snapped in half leaving the circular end (the size of a quarter) detached from the pacifier. The first time I thought it was a fluke, after two times we will be getting rid of these pacifiers. Luckily both times we caught it before my infant could grab the piece and put it in his mouth. We have had these pacifiers for about five months. The pacifier is made of hard plastic and when it hits a hard floor (ie ceramic tile, wood) it breaks the connection between the circular piece and the pacifier, allowing the circular piece to dislodge. We have only washed these pacifiers in warm water, no dishwasher. So the plastic shouldn’t have been weakened.

My 8 month old son was sleeping in his crib with a Nuk OrthoStar Pacifier. He woke up and while I was walking up the stairs I heard him choking. I ran into his room and saw he had the whole pacifier in his mouth and was choking and gaging. He vomited a little bit while I pried it loose. He is fine now.
