    Consumer Product Safety Report:

Product: Dr. Browns HappyPaci 100% Silicone Pacifier Manufacturer: Handi-Craft Company (Dr. Browns) Retailer(s): Amazon (online) Incident Date: 10/4/2025 Consumer Location: [REDACTED]

    Description of the Incident

On 10/4/25 I experienced a serious product failure involving a Dr. Browns HappyPaci 100% Silicone Pacifier. My infant was 4 weeks and 1 day old at the time of the incident.

The product had been in use since my baby was born. When she was approximately 3 weeks and 1 day old, I noticed that multiple pacifiers of the same model were beginning to tear at the base, around the small holes on the sides and near the handles. At that time, I assumed this was normal wear or possibly caused by a household pet, and I discarded the damaged pacifiers.

All cleaning and sterilization were done according to the manufacturers directions. I washed the pacifiers only by hand using [REDACTED] Baby Bottle/Dish Soap and [REDACTED] Pacifier Wipes, and I sterilized them every 3 to 4 days in a Dr. Browns sterilizer, placed on the top rack as indicated in the manufacturers instructions. The water used for cleaning was warm (approximately 100F), not boiling or scalding.

My baby had over 24 Dr. Browns HappyPaci pacifiers, so each individual pacifier received minimal use. The product was labeled as durable and easy to clean on both Amazon and [REDACTED] listings.

During the incident, while handwashing one of the pacifiers, I noticed a cat hair on the nipple tip (the bulb). When I lightly pulled the hair away using my fingers, the entire bulb detached cleanly from the pacifier base with minimal force. There were no visible signs of tearing, wear, or weakness prior to this.

If the pacifier had been sterilized and reused as usual, the detached bulb could have been a choking hazard, as my infant likely would have placed the entire piece in her mouth.

The damaged pacifier was discarded immediately after the incident, and no photo evidence was taken at the time due to the shock of the situation. The household trash was removed before documentation could occur.

Product Use Details: Product: Dr. Browns HappyPaci 100% Silicone Pacifier Quantity Owned: Approximately 24 Age of Product at Incident: 34 weeks of use Cleaning Method: Handwashing with [REDACTED] Baby Bottle/Dish Soap and [REDACTED] Pacifier Wipes Sterilization: Dr. Browns Sterilizer (top shelf), every 34 days Water Temperature: ~100F (not boiling) Usage Frequency: Rotated among 24 pacifiers; minimal use per item

    Hazard Description:

The nipple bulb detached completely from the pacifier base with minimal force during light cleaning. This poses a potential choking and suffocation hazard to infants.

    Injury or Damage:

No physical injury occurred; however, the incident presented a serious risk of fatal choking had the product been reused.

Action Taken by Consumer: Discarded all remaining Dr. Browns HappyPaci pacifiers immediately. Switched to alternative brands of pacifiers and bottles.

My 8 month old was using the Dr. Brown’s Advantage 6-18 month pacifier and got his pointer finger stuck in one of the small holes at the top of the outer hard plastic piece and couldn’t get it out on his own. Once I was able to remove his finger from the pacifier, it had a deep purple bruise and he was in alot of pain. He was in my lap at the time and I was able to act quickly, however had I not been the injury could have been significantly worse and his finger could have lost circulation completely. The product, in the report: silicone pacifier

4/26/2023: The consumer’s grandson was sucking on 1 of the pacifiers for a few minutes. When her daughter looked at the child, his face looked funny. She immediately checked his mouth and found that the head of the pacifier was torn around ½ way of the circumference. She removed the parts of the pacifier from his mouth, and he did not need any medical attention.

The baby is 6 days old and had been using one pacifier for approximately 4 days.

4/27/2023: The consumer called the manufacturer and reported the matter to them. They offered to replace the pacifier with others that they sell, but she declined that offer. The firm’s rep. (name unavailable) said they would refund the consumer’s money ($12) once she returned the pacifier to them.

The pacifiers age recommendations were 0 – 6 months of age.

The product, in the report: Dr. Brown’s Happy Paci™? 100% Silicone Baby Pacifier, Contoured One-Piece Design, Cool Gray/Green/Ecru, 0-6m, BPA free, 3-pack

I purchased Dr. Brown’s Happy Paci Silicone pacifier pack for my son from Walmart. We had their pacifiers of a similar style and they work great, but they recently changed the product name and style. We’ve had these new pacifiers for less than a week and they’re already disintegrating (we only hand washed them in warm water and mild dish soap). My husband found one that broke apart in his hands immediately with minimal pressure, leaving a piece roughly the size of a cubic centimeter. This is a choking hazard— the tip of the pacifier is perfectly sized to lodge in an infant’s airway. We checked our others and sure enough they are disintegrating too, and would have come apart soon.

The product, in the report: Dr. Brown’s Lovey Pacifier & Teether Holder (Gerry the Giraffe)

On Saturday, I picked up my 6 month old baby from his crib and I found a snap in his hand. I realized that his pacifier holder (Dr Brown’s Gerry the Giraffe) was broken and the snap had detached. My son was playing with it but fortunately I had time to take it away before he could choke on it. I can’t even imagine what would have happened if he would have swallowed it. I am very shocked that a baby toy could break so easily after only a few months. I am furious and afraid that this could happen to another baby. I would like to get in touch with that brand as we are considering pressing charges against the company. Sincerely, [REDACTED]

I purchased a Dr. Browns pacifier a little over a month ago at BuyBuy baby and the handle broke into pieces. It was the 2-pack Dr Browns Prevent Countored whale pacifier. I was at dinner the other night and noticed my 6 month old baby had pieces of the plastic handle in his hand. I don’t know how it broke off but he must have somehow been able to break it. Luckily I noticed right away or he could have choked on it. I have notified Dr. Browns and they want me to send the broken pacifier in for inspection.

I purchased a dr. Brown’s “Timmy Turtle” lovey for my daughter about a month ago. We had not owned it long, when i found the pacifier not attached to the lovey. I thought it came unsnapped, but actually the ribbon had unraveled and the snap was no longer held in place. The plastic snap came off the lovey - quite the choking hazard. See pictures. Fortunately our little one did not swallow it.

Helps to lower the pressures inside the baby’s mouth. These pressures can sometimes create dental issues such as cross bites. Palatial pressure is lessened by creating an air channel that reduces suction and spreads apart when baby is sucking. This Patented Suction-Free Air Channel opens up as baby sucks for low pressure comfort. Suction-Free Air Channel helps reduce suction and palatal pressure Soft, suction-release bulb spreads wider as baby sucks for low pressure comfort Helps to lower the pressures inside the mouth that can create dental issues, such as cross bites Thin-stem design is minimally intrusive on baby’s mouth

I bought several packs of pacifiers from Walmart for my 2 year old. Dr Brown’s was one of them. It was a 2 pack. I gave him the 1st one on a Monday night for bed. I checked it Thursday evening at bedtime & the bottom was already severed. I gave another one and it severed on the bottom side after just a few days. I’m very disappointed in their performance, not to mention how dangerous this is. I check his pacifiers all the time after reading the dangers in ingesting/inhaling a severed nipple.
