The product, in the report: Philips Avent 6-18 Month pacifier

On October 28th I found my son 18 months old, vomiting milk that he had just finished drinking, when I went to him I noticed he was having trouble getting the vomit to come out, so I tried opening his mouth...that is when I saw the pacifier completely inside his mouth, he had nearly swallowed the pacifier so far in that it cause him to gag and start vomiting. it was lodged in his throat, thankfully the ring portion was facing out and I was able to stick my finger into his throat and pull it out by grabbing onto the ring portion.

My son received Avent pacifiers for Christmas. They are the larger, 6-18 month pacifiers with sailboats on them. When replacing the pacifier in my son’s mouth after it fell out, I noticed that the nipple was almost completely torn off. Upon further inspection, the other pacifier, which hasn’t even been used yet, also has a big tear at the base of the nipple.

The product, in the report: Avent Pacifier AVENT Soother Translparent Has blue or green circle in the middle, the rest is transparent. Has a cap.

I was holding my almost 6 month old baby in my arms. She had her pacifier in her mouth. She can now grab the pacifier and put it on her mouth on her own. While I was looking at my other daughter, my baby stuck the whole pacifier in her mouth. The was no visible part outside of her mouth and she was chocking.

If she hadn’t been with me, she probably would have chocked to death. She sleeps with the pacifier and has it in her crib. This is why I decided to file a report. My daughter could have done this on her own in the middle of the night and might have choked. I do not reccommend this pacifier at all.

It is the Acent pacifier that has green or blue part in the center. The rest of the pacifier is transparent.

I purchased an avent pacifier pack of 2 for ages 0-6 months, my baby was 22 weeks. The day after I purchased the product my son had managed to get the pacifier stuck in his mouth sideways which left him unable to breathe, luckily I was in the room when this happened and was able to act quickly. My son only used these when he sleeps which is rather worrying for any parent.

The product, in the report: Philips Avent Super Soothie Pacifier. 2 pack. 3+ months. Pink

My 8 month old daughter was sitting on the floor playing with her toys when I noticed her pacifier on the floor next to her. When I picked it up to wash it, I noticed that the end of the nipple had a tear and was about to fall off. I was horrified! Had I not stopped to pick it up, my daughter would have picked it up and possibly choked on the piece falling off. I immediately went through the house and removed all pacifiers for fear of the same thing happening again. I am extremely dissapointed in Philips Avent Soothie Pacifiers. This could have ended tragically.

My son was sucking on his pacifier and turned it sideways. He inhaled it and the entire pacifier was lodged in his throat. A trip to the ER in order to pull it out with forceps. If me, the consumer, would have known or would have been warned about this at the time of purchase, I would have NEVER purchased this product. Not in a million years would i have ever thought that the entire pacifier could go down his throat.

The product, in the report: Phillips Avent Soothie Pacifier 0-3months

5 month old previously healthy infant had Phillips Avent Soothie Pacifier lodged in back of throat causing airway obstruction. The pacifier could not be manually removed. Child was brought by ambulance to a local Emergency Department and emergent circoidotomy was done. The child was then brought to the operating remove and the pacifier was removed from the airway by otolaryngology; force was needed to remove the pacifier. There was airway swelling, laceration to the epiglottis, and evidence of airway trauma. The baby remained intubated for 2 days and remained in the hospital for approximately one week secondary to poor oral intake secondary to discomfort. Baby discharged home doing well.

It is unclear how infant manipulated pacifier into his mouth. There are some case reports in the literature of this happening:

The product, in the report: Avent Phillips pacifier; 3-6 or 6+ months; blue w/ a picture of a porecupine on the button

I put my 4 month old daughter down to sleep with her AVENT pacifier. She seemed to be sucking on it rather vigorously before she fell asleep at 7pm. At 10:30pm, she woke up fussing. My husband put her pacifier back in her mouth. She spit it out within 2 minutes (thank God) and started fussing again. I went in to check on her and noticed that the fat part of the nipple of her pacifier was about to rip off the rest of her pacifier. It looked as if she had almost chewed through it. My daughter has no teeth (not even any poking through). The pacifier had been fully intact at 7pm. Had the pacifier not been spit out, she would have swollened and choked on the nipple in the middle of the night.

The pacificer was bought new at Babies R Us. We have been using it for about 3 months mostly when our daughter sleeps. She usually spits it out within an hour or 2 of falling asleep. In the past 3 months, I’ve boiled the pacifier twice (5 minutes each) and put it in a [REDACTED] microwave sterilizer bag once (for 1.5 minutes). Otherwise, I’ve washed the pacifier in cold water. I’ve never put it in a dishwasher or washing machine.

The product, in the report: Glow in the Dark Pacifier

My 8 month old son was using an Avent (Phillips) pacifier early that morning. I suddenly heard him cough as if he had something in his mouth. I turned to see if he was ok and noticed a small piece of plastic on his chest. After taking a closer look, I determined that a piece of plastic had broken off his pacifier, and must have ended up in his mouth. The broken piece was the center section of the handle. There is a clean break in 2 locations, leading me to believe a manufacturing defect in this component. Aside from the obvious CHOKING HAZARD GENERATED BY THE SMALL PART, the edges where the plastic severed is very sharp and could have cut his skin. We now have numerous Avent products, including pacifiers, nipples, teethers, and bottles that we are uncomfortable using. These goods cost us well over $100 and will need to be discarded and replaced for my wife and I to feel our child is safe during use.

My son could have been severely injured, or even worse could have died if were not close by. I am concerned for other children using these products. I feel they are unsafe; something needs to be done by the manufacturer to rectify this, and potentially many other situations.
