The product, in the report: Baby stroller
My 8 month old baby was sitting in a Graco Outpace Baby stroller and we turned to go the opposite direction and the stroller completely tipped over and my grandson hit the street pavement. I did not hit anything or turn aggressively, just the act of turning tipped the stroller over.

The product, in the report: Graco Outpace LX Stroller
I have owned a Graco Outpace LX Stroller since 2024 and this stroller is not safe for toddlers at all. It has tipped over 3 times with my child strapped inside of it and yesterday caused an extreme injury and goose egg on my child’s head. We REFUSE to ever use the stroller again since it is unsafe although I spent quite a bit on it.

Seen by a doctor or nurse Made Nov 21, 2025, sent to the company Nov 25, 2025, published by CPSC Dec 10, 2025
The product, in the report: This is a Graco travel system stroller, purchased on November 23, 2023. It is a full-size stroller with a seat that can be used in a reclined or seated position. The stroller is black in color and includes a removable seat pad and adjustable harness. The backrest of the seat contains a metal screw located in the center of the lower back area underneath the fabric padding. This screw is slightly raised, creating pressure contact with the childs lower back during normal seated use. The stroller is intended for infants and toddlers as part of a standard travel system.
My one-year-old child developed a recurrent abrasion and bruise on his lower back after using a Graco stroller purchased on November 23, 2023. When we began using the stroller in the seated position, we noticed a small round injury that always aligned with the screw located inside the seat back area of the stroller. Each time we used the stroller, the irritation worsened; when we stopped using it, the area gradually healed. A bruise has remained visible, and a small scar has formed.
Our pediatrician examined him and documented: Superficial healing abrasion over lower, mid-back with small underlying bruise, and noted that the recurrent abrasion and bruising correlate with stroller use. No other products the child uses have any protruding hardware in contact with the back.
This appears to be a possible design or assembly issue involving the exposed screw in the seat back area. The injury occurred during normal and appropriate stroller use. I am submitting this report to request review for potential safety risks to other children. I am willing to provide the stroller for inspection if needed.

The product, in the report: Modes Nest Stroller/Car Seat System
During a walk using our Graco Modes Nest stroller system, the car seat detached and tumbled off the stroller unit with our baby inside of it. Thankfully, our baby is mostly fine (just a scratched knee), but this could have been much worse. We’ve struggled with the attachment points between the car seat and the stroller since we purchased it, but this was the first time it was secure that something bad happened. I think it’s because there is fabric of the bassinet that covers the attachment points of the stroller, making it difficult for the car seat to be secured.

The product, in the report: Stroller w/ detachable car seat. Graco Verb Travel System in color merrick
I was putting my son in the car but reached into my car to grab something first. I left his stroller on my driveway and made sure to lock the brakes. My driveway is pretty steep but he was fine because I assumed the brakes worked. I went into my car and then saw the stroller rolling down the driveway despite having the brakes on. I then ran for the stroller and fell which resulted in me fracturing my shoulder and Im all cut up. The stroller crossed the street and luckily my neighbor came running to save him and then help me because I couldnt get up.

Graco Fastaction SE 2.0 Travel system Astaire Purchased Albany OR Target Feb 4/2024
Reporting serious product failure 7/9/2024. Locking mechanism that keeps stroller upright and erect failed during use. Pushing stroller at walking pace from parking lot, pushed stroller across threshold of the building stroller folded while in use carseat containing child tipped forward and upside down as it collapsed to the ground.
Child appeared unharmed, though he was upset. The child was bucked in according to instructions. If he had not been buckled, or if the carry handle had not been in upright position, this could have resulted in serious injury.
Graco webform was experiencing technical difficulties when I tried to report there July 10. Customer service line went unanswered for significant time.
Feeling very frustrated.

No one hurt Made Sep 19, 2022, sent to the company Oct 3, 2022, published by CPSC Oct 18, 2022
The product, in the report: Graco® Modes ™M Pramette DLX Stroller in Hamilton
I purchased my stroller on April 2, 2022 and had my son August 18, 2022. When I put my stroller together and laid my son in it for the first time 2 weeks ago I quickly realized there is a huge safety hazard. Graco promotes this stroller to have a bassinet option which they call “Pramette” which allows parents to lay their newborn down in the stroller as they go on walks. The first time I did this my son slid down due to the product not lying flat, and could have seriously become very injured. In this option the stroller also rocks back and forth very unstable, which for a newborn could cause serious medical problems. I quickly took my child out and read the instructions thinking we did something wrong, to my surprise it was set up exactly as Graco states. I contacted Graco very concerned and frustrated that this product was not working as advertised, that my child could have been seriously injured. Customer service is aware of the poorly designed stroller, understands that this part doesn’t lay flat to secure a newborn, and stated if I wanted the product to lay flat for my child’s safety I can try buying an additional part that costs $100+. Graco understands there is a safety hazard and they could have injured my child and many other children as the reviews on this product are shocking, and they can care less!

No one hurt Made Apr 25, 2022, sent to the company Apr 28, 2022, published by CPSC May 12, 2022
The product, in the report: Graco Fast action fold SE travel system
I was walking my baby in his Graco Fast Action Fold SE stroller/travel system. He was in the carseat connected to the stroller. We had been walking for around a hr, and stopped to eat lunch. I fed him half a bottle and strapped him back in. About a half HR later I stopped again to feed him the rest of his bottle. When I put him back into the carseat, I didn’t even get a chance to strap him in, as the stroller buckled ,collapsed, and folded. Sending my loose baby and his carseat flying forward onto the ground. I was mortified, not knowing if my baby was ok while running to pick him off the ground. I rushed him to the hospital, and he was fine besides the shock. This is so very dangerous, and had we been on concrete or cement, I am a hundred percent positive he would have been seriously injured or worse. Needs to be recalled ASAP.

The product, in the report: Graco FastAction SE Travel System. Includes Quick Folding Stroller and SnugRide 35 Lite Infant Car Seat.
Purchased the Graco FastAction SE Travel System for my newborn baby. I was walking my 4 month old in this system. He was in the car seat, strapped in… I went over a small crack in the road and the stroller completely folded upward in its stow away position. The stroller fell with the car seat still attached to the stroller. The car seat went down head first. My baby did not hit the ground but the car seat hit the ground pretty hard. When I got home I called the pedi to check in on my son, and report the issue with graco. There many reports online reporting this same exact issue. Not sure why product has not been recalled. I am no longer using this system.

The child (age 7 months) was strapped into the the back stroller seat appropriately. There is no shoulder strap--only the lap belt. Mom was pushing the stroller and became worried when the child did not respond to her. When she stopped the stroller to look at her child, she noted that he had flipped forward and his head had become lodged under the seat of the first stroller (this is a double stroller product).

The product, in the report: Lightweight, compact stroller
My family and I were fishing, my son was in his Graco Jetsetter stroller. I stepped away for a second after putting on the brake and all of a sudden I look up to see the stroller is rolling towards the lake with my son in it! I was able to get to the stroller before my baby was dumped into the lake but after inspecting the stroller noticed that the brake was still on. Later the same day at a different park about 5 hours later my son was once again in the Jetsetter when he leaned over and the stroller fell on its side. Had my son been on a hard surface he could’ve been serioously injured. The stroller brakes seem to be too cheaply made to hold much and the stroller itself is obviously top heavy. My son could’ve been killed today had I not been paying attention and been standing close by

On August 26th 2018, my husband and my two children where shopping at about 9:45AM in Lancaster, PA. We unloaded our double stroller, ensured it was locked, and placed my daughter, who is 10 months in the front seat and my son, who is 3 years old in the back seat. We knew the stroller was locked before placing our children in it. We started off and my husband was pushing them for about 200 yards. While walking, when we hit a slight downward grade, the stroller began to fold. It folded so fast where my husband had to jump over it to avoid falling on top of the stroller. The stroller fell forward collapsing on my 10 month old daughter, and my 3 year old son. My daughter ended up with a broken femur, while my son came out with no injuries.

I purchased Graco Ready to Go Click Connect Travel System from Toys r us. After buying 3 differnet models of the same item, I have realized that this product is an extreme saftey risk.
When my son was born and we brought him back from hospital, on the leveled floor of Hospital, stroller started shaking. It turned out to be the front wheels. I tried different items (same model) at Toys r us at Madison, WI and the store rep and I came to conclusion that all items were defective.
Issue is with the front wheels. They rattle and cause shaking movement. Also wen you run with them on a flat service (for jogging), wheels roll and shake really bad.
I reported the issue to GRACO but all products are bad. They sent me 3 replacement wheels and all turned out to be bad and the issue is still there. I have videos and pictures of the issues.

I am taking it back but don’t know how or where. I am also in the process of a lawsuit but don’t know if it will go anywhere. I am warning everyone I know not to buy Graco. Also, my daughter cut her finger in the crook of the tray when it was to the side and she tried to lift it. I have to say though, the stroller is very sturdy and durable and to this day I would still recommend Graco. Please fix the issue with a parental warning on the product booklet.

The product, in the report: Graco LiteRider stroller
In October 2014, we were pushing our 1 year old daughter in the Graco LiteRider stroller when my husband twisted the handle by accident. About ten seconds later, our daughter started screaming in pain. We soon realized her finger had been caught in the side hinge that releases when the handle is twisted. Her finger was smashed, got extremely reddish purple in color, and was hot to the touch for a few hours. We iced it and it got better as the day progressed so we did not seek medical attention, however it was almost a very serious situation and thankfully we realized what happened as soon as we did and were able to relase the hinge.

The product, in the report: Graco Ready2Grow Click Connect Stand & Ride Stroller - Onyx
Graco Ready2Grow Click Connect Stand & Ride Stroller - Onyx (Babies R Us) When placing the Graco Click Connect infant seat into the stroller, the seat would not sit level. (car seat facing stroller handle in the rear seat position) The seat clicked into place, however the the side support pieces never clicked into place simultaneously. If the left side was in, the right side was out and vice versa. This left the car seat tilted to the right or left, causing the baby to be leaning into the same side.
After experiencing this with my stroller, I returned it to Babies R Us and asked them for a new one. Before I left the store, I asked them to open the stroller and place one of their Graco Click Connect infant seats in the rear seat position. The store’s stroller/seat combo also had the same problem.
I ultimately returned the stroller instead of exchanging it for a new one because I do not feel safe having my infant in a car seat/stroller at such an unstable angle. I feel this is a design flaw of this stroller and do not want a child falling out of the infant seat due to this issue.

The product, in the report: Graco LiteRider Classic Connect Stroller, Dragonfly 3-point harness
Graco LiteRider Classic Connect Stroller, Dragonfly-
This stroller does not have a 5 point harness. It is only a 3 pt harness. Only the picture lets you know that when buying. It is not listed on the box anywhere. This is our first child and I did not think a three point harness would be dangerous.
After using the stroller, my nine-month old son TIPPED THE STROLLER OVER because the stroller has no shoulder straps and when he leaned over to the side, the entire stroller fell over. WHY MAKE A STROLLER WITH NO STRAPS!?!?!?! Children really need shoulder straps. My nine month old hurt himself very badly because of this stroller-he has a bloody face now.

The product, in the report: Graco Ready2Grow Double Stroller
I used the Graco Ready2Grow double stroller today with my twin daughters. I used it as a regular stroller for the first time and did not have the infant car seats snapped in. So, I used the straps in the stroller. When we got home from our walk, I could not get one of my daughters out of the stroller. The strap snap would not release no matter how hard I pressed the release button. I attempted for 10 minutes to get her out and could not get the button to release. I ended up cutting the straps to get her out of the stroller because at this point she was screaming. Thankfully, we were not in a life threatening situation but I think that this could be problematic if a parent was needed to get their child out fast.

The product, in the report: Graco Click Connect Ready2Grow double stroller
I was taking my two children, ages 3 and 5 months, for a walk for the third time in our new Graco Ready2Grow LX double stroller. I had them both buckled in securely and the stroller felt stable, when it suddenly collapsed almost all the way into the closed position! My 3 year old was pinned underneath it and was crying and terrified. Both children had scrapes and my older daughter had a bump on her head. I feel it could have been much worse if my 5 month old was a little taller and the tray would have struck her in the head instead of scratching her as it went over her. It was very frightening. The stroller had been open all the way and definitely seemed to be in a locked position before I strapped my children in.

His arm became caught in the rear sunshade. This resulted in a swollen arm and hand along with several remaining bruises. In addition, the arm is covered in Petechiae from about 2 inches above the elbow to the wrist and hand. He does not seem to be experiencing loss of motion at this time. We are monitoring him for further signs of damage.
I want other parents to know of this risk so this does not happen to their child. Thank you!

The product, in the report: Graco Ready2Grow LX Odyssey stroller, sit and stand childrens stroller
While my wife and I were shopping for a new stroller @ babies r us, we were considering a Graco Ready2Grow LX Odyssey. After getting acquainted with the operation and similarities to our other Graco stroller we decided to put both of our children in the seats and try it out around the store. When my wife began to push the stroller down the aisle it suddenly collapsed into the folded position unexpectedly. Our youngest son was struck across his chest and neck area by the tray of the rear facing seat, my wife had lost her balance due to the sudden unexpected collapse and was struck in the ankle with the rear axle. My older son (sitting in the front seat) was contorted into an awkward position while he was suddenly cast towards the floor. It appeared that the only reason this stroller had not completely folded into the storage position was because our children were in the way. Neither of our children are any where close to the listed safe weight limit, and we could not find any obvious flaw or reason to suspect such a failure as we had owned a very similar model previous to this incident. The manager took an incident report and offered ice and bandages and instructed us that someone from their company would be contacting us soon.

The product, in the report: Graco Alano stroller Graco Alano Travel Systems-Meadow Menagerie model number-1771730
My 7 months old son was sitting in the stroller. He was biting on the tray that is attached to the stroller. While he was biting the tray his tooth got caught in the slots on the underneath of the tray. When his tooth got stuck he fell back away from the tray which pulled out his baby tooth. There was a call placed to his doctor’s office because he was bleeding a lot. I rushed him into the doctor’s then I had to bring him to the emergency room where they pulled out his tooth due to the fact that his tooth was hanging by the gum so he didn’t swallow his tooth.

The product, in the report: Graco Quattro Tour Duo double stroller; Brown, Tan, green
On February 29, 2012, my wife was teaching a class at [REDACTED] and our one and two year old boys were in the Kids Club. My wife brought our 2010 Graco DuoGlider Double Stroller in for our one year old to take a nap in. Our two year old was trying to get something out of the undercarriage basket and proceeded to reach his head and arms into the basket. While in this position, he walked his body around to the side of the stroller to get a better reach on whatever he was trying to grab and his head got stuck between the frame underneath the back seat and the undercarriage basket. His head was stuck in an approx. 6-8 inch gap. The amazing kid’s club staff realized what was happening immediately and jumped into action. It took 2 staff members and breaking the basket to remove him. During the minute and a half that he was stuck his airway was compromised and he stopped breathing. When the staff removed him he was blue and they were able to get him to come to within seconds and he began to cry. Their fast-acting, life saving efforts saved our son’s life. After an ER visit and thorough check-up our son was fine with just some scraps and bruises.
