The reports, as CPSC published them

Reports people made to the U.S. Consumer Product Safety Commission. CPSC publishes each one after sending it to the company named, and doesn’t guarantee they’re accurate. A report isn’t a recall.

Each report is the words of the person who made it, as CPSC published them. The company’s comments, where CPSC published any, are set apart as the company’s own.

The product, in the report: Jumper, exer saucer with a seat that turns 360 degrees. Toys surround the seat on a tray, and it has two hanging toys.

We just bought this saucer on Friday for our almost 8 month old granddaughter. She first used it on Saturday under close supervision. It seemed like she was almost able to tip it over even though she could barely touch the saucer, so we raised it to the next higher position. On Sunday night, even though she was being closely supervised, she pulled herself out of the seat, and fell over the side onto her head. We rushed to her, and she had a large bump and red mark on her head, but other than being scared, she seemed ok. We raised the seat to the highest position again, thinking that if she couldn’t touch the bottom, she wouldn’t be able to push herself over the top. While she was being supervised today, she almost went out on her head again, but this time we got to her before she fell. She is a very small 8 month old, and just was 8 months old yesterday. She was just able to sit up on her own this week, and isn’t crawling or pulling herself up to stand yet. It was very shocking that she was so easily able to pull herself out of the saucer and tip out onto her head. Thankfully she wasn’t seriously injured. We had exersaucers for our two youngest children, and never experienced a safety problem with them in the past, so this is quite shocking.

The product, in the report: Exersaucer Triple Fun™ Animal Planet Activity Center

My youngest child is 6-months-old, 18.5 lbs and 27 inches; which is within the age, height and weight restrictions of his exersaucer. He was seated, playing in his exersaucer in a onesie and diaper with no pants playing. When I went to lift him out of the exersaucer his left leg seemed to catch on something and he screamed. I didn’t initially see how he could have scratched himself on his upper thigh. Later the same day he was playing in the exersaucer and once again when I lifted him up his left leg seemed to catch on something and he screamed and cried. I looked under his exersaucer and the design of the bottom part of the plastic portion that holds in the fabric child’s seat has these almost U-shaped cut outs that have rough edges and when my baby lifts his legs up when I pick him up, it allows his bare legs to get caught and scratched. I do have a couple of pictures of my son’s legs extended and pulled up so you can see how easily his legs hit and can get caught in this rough plastic.

The product, in the report: Evenflo Beach Baby Exersaucer Model 61611369 mfr. date 2015-02-16 PQ 1

As reported to Evenflo (which they informed me “We do not this product anymore” by voicemail.

Chat Conversation Start APR 14TH, 10:48PM I would like to provide some feedback on some of your exersaucers that should be taken seriously. I believe you should consider recalling some of them and replacing one of the attached toys. My son leaned forward in the “Beach” Exersaucer without the stands in place and the cup, yellow orange featured in the picture below became lodged at his neck. It actually choked him and I’m happy that I was supervising hid play at the time because I cannot imagine what may have happened otherwise. He had not exceeded the height limit so I’m sure this could happen to some one else as well. Once is enough. The stands being off only made it more difficult for his body which as a baby he has limited strength, to pull himself back up and off of it. Please take this seriously. We are no longer using this item but I fear from this experience that it could easily end much worse, especially if a parent were unaware, even for a moment. It’s extremely dangerous, a potentially fatal hazard given s babies limited strength, and something that whoever reads this really needs to forward to the appropriate person to handle accordingly. I have not contacted CPSC but I have considered it due to the serious nature of this hazard. I’ve typically been happy with your products and I realize you may be unaware of the danger this particular piece and design poses. Please see the image below, even the pictured baby appears to be right at the right height and position that if she were to lean forward the yellow piece could choke her. It’s a straight plastic line and with the stands off it would be difficult for her to lean herself back off of it. And our experience was terrifying. I realized how easily a child can be can severely injured by a toy. Sadly too many children have suffered loss of life from items that parents thought were perfectly safe for them.

The product, in the report: Exersaucer - Bouncer

This caller is reporting his 8 month old daughter’s exersaucer/bouncer purchased in December 2014 from local retailer. The caller’s 8 month old daughter was sitting in her exersaucer while the caller/father was sitting next to her with his other daughter when the baby began crying. He jumped up and ran to her and saw that the baby’s finger was caught in lace type rope/string holding the cake toy together. The baby’s finger was swollen and her father had to cut the cords from around the baby’s finger, the father states that he was pretty much in shock but believes that the injury is to the baby’s right thumb. The father treated the baby’s thumb at home and treated her. He has not contacted the manufacturer but does intend to contact them. The caller is asking that the public be warned about the hazards of this product since there are more of these cords/strings on this unit, and that this exersaucer become recalled before other baby’s are injured and that replacement of product be available at manufacturer expense only.

The product, in the report: Evenflo Doorway jumper. A spring seat that attaches to a door jamb for infants to jump up and down

While using an evenflo doorway jumper at a little over four months old my daughters arm became entangled in the leg hole. This forced her neck directly onto the shoulder support bar and she began to choke. She did not have the coordination to get unstuck. After the incident I double checked the manual’s requirements for use and she fit all of them. I immediately took the product back to the store and contacted the company. Apart from an automatic customer response email they never got back with me. The requirements for this device need to be updated for an older infant.

The product, in the report: Evenflo Exersaucer JungleQuest Jumper. Has a 3 post connection in bright lime green colors with animal attachments on top of each post. The post connect to the bouncy seat in the middle using a bungee cord method. the seat in the middle swivels and is surrounded by 6 different activity toys the babies can play with. At the base of seat there is a foamy plate that the babies bounce on and can push off with their feet

my son is 6 months old and weighs 16.5lbs and was bouncing and playing in the exersaucer junglequest jumper early morning like he has done several times before. The jumper has 3 connection polls that use a bungee cord method to attach the poll to the bouncy seat in the middle. As he was bouncing on it, one of the bungee cord connections came off and unattached from the seat in the middle which he was sitting in which made my son fall out of the seat onto the floor and metal circle part that attaches the polls together at the base. His head, face and upper body lay diagonally on the floor and his legs still in the seat. His head and face hit the floor and metal circle at the base. He started screaming and crying. He has never fallen out of anything before. He was very scared and I wasn’t sure if he was hurt or how bad it was. My main concern was that he had hit his head, so I rushed him to the emergency room to have doctors look over him and thank God he’s ok. the doctor just told me to watch him throughout the day for any signs or symptoms due to head trauma. This was a very traumatic experience for my son as well as for myself. I will think twice before ever buying any product from evenflo and definitely will not be recommending any evenflo products. Evenflo should definitely come up with a different method as to attaching the seat to the connection polls because its apparent that there current method cannot hold the connection. I wouldn’t want any other parents to go through this

My 4 month old daughter was in the evenflo exersaucer “123teaforme”. She was pulling the hat with the mirror towards her to try and get it in her mouth, with one hand grasping just under the hat and the other hand down by the orange base, her fingers at the bottom of the pink stem in between the 2 round plastic pieces. As she pulled it towards her mouth, her index and middle fingers were pinched between the plastic causing cuts on the top of both fingers, and lots of tears.

The product, in the report: Evenflo ExerSaucer® Double Fun™ Pink Bumbly Saucer

I received an Evenflo ExerSaucer® Double Fun™ Pink Bumbly Saucer as a gift for my daughter whom is just 6 months old now. I am happy with the exersaucer but there is a problem with it that I find very dangerous. On an earlier date I when pulling the baby/infant out of the seat I noticed that her foot was deeply scratched. Not to the point of bleeding, but to the point of discomfort. On the actual date listed, I took the baby/infant out of the exersaucer and her foot was badly scraped/scratched again. I now realize that the ring that holds the fabric seat/sling part has sharp edges and cuts.

I took the seat apart and literally cut my finger on the roller ring due to sharp plastic sticking out and the edges being far too sharp for a baby toy. If they had the seat/sling fabric attach to the ring portion on the inside as well it would eliminate much of the risk. The problem is that the fabric seat/sling attaches on the outside of the ring and when pulling the baby/infant out of the toy it turns inside out exposing the inside of the ring to the baby’s legs while pulling them out.

I feel that this is just bad safety for a baby toy. All plastic edges should be smoothed and made certain that they will not injure a child. I wonder if all the units are made with such disregard for safety. It is clear that this one was sent out knowing it was like this because no one is considering the fact it is a danger while pulling the child out of the toy.

The product, in the report: Brand name - evenflo make/ model - exersaucer baby active fall hazard due to structural design of pillars

Evenflo exersaucer baby active fall hazard. My 6 month old daught was in her bouncer and the tabs the lock the 3 pillars in place had given way. The pillars popped out of the base of the exersaucer and the lower portions fell inward while the weight of the upper portion of the exersaucer and my daughteds weight fell down nearly crushing her lower body. Shes not incredibly active so she doesnt stree the locking tabs as my son had done with the one we bought for him. Im a mechanic and insist on double checking my work when assembling everything. Approximately 3 months have passed with this object was being used. I ensured the tabs and other locking mechanisms where properly in place befor use, and as I had put it back together when it broke, the tabs would not lock, then it fell apart without the babys weight. This product is unsafe as the structural design is only made to last a short time period. Please figure this situation out befor any children get hurt.

The product, in the report: Excersaucer triple fun

This excersaucer has three stages. The third stage is for an older baby that can stand and play with toys. The toy turns into an a S shaped platform on 4 legs. When my daughter played with it it has tipped over on two occasions. I have looked online and found many other people that had the same problem and was to make home fixes. When I called evenflow they told me “ the child should not put their weight on it”, in theory that makes sense but explain that to a 14 month old baby, not very realistic to expect a baby to not lean on a toy while playing. This should not be on the market

The product, in the report: Hard plastic, children’s activity center with small detachable parts

The baby was bouncing in the activity center (set in the rocking mode) when suddenly the baby leaned forward and the tray tilted over. The baby came out flying out of the activity center. He fell on the floor face down, hitting his forehead and losing his breath. The baby’s grandmother picked him up and started blowing air into his face. “The consumer specifically indicated that she blew breaths of air over the baby’s face.” After a few moments the baby stated to breathe again. The baby had a big bump on the left side of the forehead. The consumer proceeded to take the baby to Saint Christopher’s Children’s Hospital, Philadelphia, PA, right after the incident. The doctor diagnosed the baby with a mild concussion.

The consumer will provide more medical information when she receives the report for her review. Notes: This product has been recalled (CAP# 09-131) according to the model number and when it was manufactured. The incident type is different from the ones described in the recall notice, since the activity center tilted over.

The locks that lock the legs in plac e at stage 3 DO NOT WORK PROPERLY. It was bought for my 6 month old where there is also a 2 year old in the house. The two of them played with it for 15 minutes and it had fallen down and the 6 month old was crawling underneath when it happened. She got pretty badly hurt and we took the toy away to ensure it was put together properly again. My sister owned the same unit for the 2 year old in the house and had no issues with it that is why I bought the same model. We got the locks to finally lock and STAY LOCKED but after a few days we seen small springs beneath the table. THESE CAME OUT OF THE LOCKS THE CHILDREN COULD HAVE CHOKED ON THEM. We decided for the safety of the children not to let them use it anymore and turn it back into stage 2 instead of stage 3. At that time the monkey toy snapped off the plastic clipping that is attached to the bar was half inside the toy and half outside the toy with sharp pieces of plastic on the ends. Upon taking the toy off we seen inside that the spring that holds the legs up and gives the children the ability to jump was warped and pushed off the she side. THIS TOY IS TERRIBLE MY CHILD HAS OWNED IT FOR LESS THEM 6 MONTHS AND I WOULD NEVER RECOMMEND IT TO ANYONE AFTER THIS. THEY WILL NOT DO ANYTHING ABUT WHAT IS BEING DONE. I WILL NEVER EVEN BUY THERE PRODUCTS AGAIN AFTER THE INJURIES MY CHILD HAS HAD FROM THIS TOY.

I purchased the evenflow Exersaucer this afternoon for my grandson. We where in my living room when I put him in it. within moments he started to play with the block building. With in seconds he started to cry. he had pinched his fingers between the blocks. I soothed him & let him try again thinking it was a fluke. With in a few seconds he did it again. I removed the building. Then he chose to play with the kite. he grabbed hold of it & was about to stick it in his mouth when he lost his grip & it popped him in the nose, resulting in a bloody nose. I returned it to target. 3 minutes of play resulting in 3 injuries is insane.

The product, in the report: Evenflo Mega Splash Exersaucer. It’s blue and orange in color and has toys on the tray.

The Evenflo Mega Splash is an UNSAFE product! The legs become misaligned from the bottom base and the legs pop out and the springs fly off and the whole top comes off! I’m lucky that my son was not in it when it happened yesterday. I just grabbed it to move it and the whole thing fell apart. I look online and this is a common issue with this product. PLEASE do something so another child does not get hurt in this product!

The product, in the report: EVENFLO 3 in 1 exersaucer (jungle theme)

SUBJECT: Evenflo exersaucer 3 in 1 (A 3 stage toy - meant to grow with baby)

I have had numerous issues with this product.

I did not use stage 1, as the arch over the child moves very easily. i dont know that it can be dangerous as i simply preferred not to use it.

The exersaucer stage is great - except for 2 things: 1) The sound/music level of the toys is very high - even on the lowest setting. I cannot imagine that it does not damage a baby’s hearing. 2) There is a dragonfly toy that the baby can pull at. This poses a risk of injury to baby’s face, as baby movements are ‘jerky’ & they can easily hurt themselves with the edges of the dragonfly wings. Its especially scary when it comes close to the eyes)

The stage 3 walking stage is HORRIBLE & VERY DANGEROUS!!!! Besides for the same issues above (as it uses the exact same toys): 1) It is EXTREMELY SHAKY and FALLS ON TOP OF CHILD when tries to stand up against it [this happened to my child TWICE in just the first couple days of changing to this stage] 2) the ‘locks’ (that are meant to keep the legs from turning or collapsing) have tabs that stick out. If a child even lightly presses these tabs (can EASILY be done accidentally, just while paying with the toy), the tab will fall ALONG WITH A SMALL SPRING that is 100% - a CHOKING HAZARD!!!!!!! This toy MUST be recalled BEFORE it claims the lives of innocent children!!!!!!!!!! [this happened a couple times too - altho Thank G-d a million times over - I saw it happen immediately & was able to take away from my child]

Needless to say, after only a couple days, we already dis-assembled this product!

The product, in the report: ExerSaucer® Splash™ Active Learning Center, Bubbles (also know as: Evenflo Splash Mega Exersaucer) Model Number: 6161948 ASIN: B002VUCBC2 UPC: 032884161768 Product Dimensions: 28.5 x 9 x 31 inches ; 13.3 pounds Product Description From the Manufacturer The ExerSaucer mega splash stationary activity center provides baby with a beach-themed learn and play experience. A variety of toys help baby achieve important developmental milestones such as gross and fine motor skills, self-awareness, tactile development, and more. As “a safer alternative” to walkers, the Exersaucer provides baby with the benefits of stationary exercise with rock, spin and bounce capabilities. A removable, machine washable seat pad in a farm print provides for easy cleaning. Three height adjustments accommodate the growing baby.

My INFANT daughter was playing in her EVENFLO EXERSAUCER MEGA SPLASH ACTIVITY CENTER. She was turning and bouncing in it like she always does and one of the top legs came off of the top base-causing her to fall forward. Thankfully i was there to catch her and she wasnt hurt. There is no hardware included or required to assemble this product. The bottom of the legs attach to the base pretty securely. They pop in and sort of click in place. The top part of the leg attaches by matching it up with a notched section on the top, pushing against the spring, and turning it so it doesn’t line up with the notched part. There is not anything that fastens it, or prevents it from turning back the way it went in. After the incident I decided to search any recalls for this product AND THERE ARENT ANY- but there are quite a few people who have had the same thing happen with their child. Look at these reviews:

HEALTH RISK! Danger to your child, December 19, 2010 By [REDACTED] - See all my reviews This review is from: Evenflo Splash Mega Exersaucer (Baby Product) I bought this for my son when he was about 5 months old. It worked fine, for about a month, when he was just starting out. He (like most other baby boys) is an active little boy. When he would start bouncing in the exersaucer the legs would begin to re-align so that the tabs would unhook and the top part of the saucer would become disconnected with the leg. Two of the legs did this WHILE HE WAS IN IT! He was only 6 months at this time and thank goodness he didnt get hurt but the top part falls down (diagonally to the floor) allowing the baby to fall forward. It is a serious health risk and I’m so thankful and surprised he did not get hurt. My husband thought it was a fluke, put it back together and the next day after numerous bounces once again it fell apart literally! Right while he was in it. It was well put together, there was no issue with installing it properly. It was not defective in any way. Bought brand new from the store. Its just after a few days of bouncing in the chair the orange pieces spin just enough to align themselves perfectly to become dis-joined. No wrongful use of the saucer to cause it to come off. Just poor manufacturing. I recommend to buy something where the top half connects to the bottom without a simple align & twist. Maybe a snap together instead. I got the [REDACTED]and we’ve been happy ever since.

1.0 out of 5 stars Dangerous--leg collapsed, April 20, 2011 By [REDACTED] - See all my reviews This review is from: Evenflo Splash Mega Exersaucer (Baby Product) Our 5 month old daughter loved playing in this but one day one of the blue caps popped off, the spring went flying, and the leg collapsed. Luckily I was standing right there and my daughter was not injured, but she really could have been hurt! I would not recommend this product--it’s not worth the risk of injury.

Newer model isn’t as good - missing feet!, June 30, 2011 By [REDACTED] - See all my reviews This review is from: Evenflo Splash Mega Exersaucer (Baby Product) I was totally swayed by the new colors and fun toys for this exersaucer. I had the farmyard one for my daughter 3 years ago and loved it so much. I had shared it with friends and lost track of it, so when it came time to get one for my 5 month old, I chose this one. I was so excited by it, but quickly realized it wasn’t as good at the older models.

First off, it doesn’t have stabilizing feet. It is way too wobbly for a younger baby. I ended up buying feet from ebay to fit onto my Evenflo Splash Mega Saucer. Those worked a little better, but they aren’t perfect. Why would this product have space for the feet, but not have them? I hate how toy/baby companies do that. They lure you in with a product and then get cheap on you, by removing features.

Secondly, the older model collapsed. They may not do that anymore as a safety measure, but I liked that we could take in our car to our friends’ homes.

Thirdly, we had a 6 month old baby come over last weekend to play and we put him in it. He managed to rip out the switch-a-roos. The only one that didn’t rip out was the apple switch-a-roo that we bought as an addition. That never happened with our older model farm exersaucer.

    So disappointed. Hope this helps!

Looks great, but its a bad product., October 25, 2011 By [REDACTED] - See all my reviews Amazon Verified Purchase(What’s this?) This review is from: Evenflo Splash Mega Exersaucer (Baby Product) I bought this product for my 5 month old son because it looked really nice in the pictures and my son loves playing with toys and jumping. When I received it, we set it up and it worked well for maybe about a month and then my son starting jumping in it and one of the blue caps popped out went down, and its NOT safe for your baby to be in if there is a older child around that likes to mess with things and who are curious about pushing buttons. My niece was curious about it and pushed it, and went down easily. I prefer something that is child proof. Also one of the springs came off. Also when he was playing in it, I noticed that he tore one of the paper labels and was chewing on it, I took it away. I think they really fix stuff on here especially the legging and the paper labels. I think its better of buying something else more expensive then buying this product because your baby could get hurt or swallow one of the paper labels.

Waste of money!, June 14, 2011 By [REDACTED] - See all my reviews This review is from: Evenflo Splash Mega Exersaucer (Baby Product) I had this for 6 mos and all 3 springs, (which helps the toy bounce) popped out. So angry. I would give it 0 stars if I could.

my baby chews the paper labels, October 9, 2011 By [REDACTED] - See all my reviews This review is from: Evenflo Splash Mega Exersaucer (Baby Product) My baby has been using it for a couple months now. Everything worked well except yesterday I noticed that she had eaten some of the paper labels on one of the toys. I can still feel the glue on it. I don’t know what the papers and glue are made of. Please be careful with it if you plan to get one.

This was great until it collapsed with my baby in it, April 3, 2012 By [REDACTED] - See all my reviews Amazon Verified Purchase(What’s this?) This review is from: Evenflo Splash Mega Exersaucer (Baby Product) First let me say that it was definitely assembled properly, and my son has used it and loved it since Christmas without incident (so about 3 months). This has several pieces, including the top, the legs, and the base. The legs contain springs which make it potentially very bouncy if you have an enthusiastic baby (which I do).

I think this is what happened--after some use with an occasionally enthusiastically bouncing baby, the piece at the top turned slowly and worked its way back toward the notched opening. Luckily I was 2 feet away when it happened and he wasn’t hurt, but one leg popped off entirely, one detached from the top and one detached from the base. It would have been pretty ridiculous if it wasn’t so alarming--the thing just quite literally fell apart with him in it. The leg that stayed attached to the base was a few inches from hitting him in the face with rough edges as he fell. He could easily have had a leg pinned in the wrong direction and had it hurt by the weight of his body pulling down on the top. What a disaster. He was terrified but unhurt, thankfully.

If you buy this for your child, please periodically check under the top to make sure the legs have not twisted back toward the notch in the top to prevent this from happening again!

PIECE OF [REDACTED] ...DANGEROUS...DON’T UNDERSTAND ALL THESE GREAT REVIEWS!, July 8, 2010 By [REDACTED] - See all my reviews This review is from: Evenflo Splash Mega Exersaucer (Baby Product) I bought this exersaucer just yesterday after giving the Evenflo Smart Steps to my in-laws since they watch my son during the day. He’s now 6 months old. We put it together, did everything right (I’m a nervous new mom and VERY careful) and one of the legs collapsed and my son nearly got hurt. I have trust in Evenflo since I love the Smart Steps Exersaucer but not any more!!! I returned it and exchanged it for another one thinking, maybe this particular one is just defective....but this one collapsed as well!!!! Same thing, one of the legs just came loose. Again, my son was almost seriously hurt! I cannot even believe they have this on the shelves. I’ll be calling Evenflo to complain and make them aware of the danger involved with this product! Please spend a little more and get a different one for the safety of your child!

9-month old baby falls out of Evenflo Exersaucer Mega, manufactured on Feb. 1, 2007. Baby was on tippy toes and leaned herself over and fell out. Baby hit head on the floor, luckily diagnosed at the ER with minor acute closed head injury. Baby is 28” tall.

This product unit does not have any indication printed on it as to when baby must discontinue use of product based on baby’s height. The only information printed is to discontinue when baby is walking.

Product manual says to discontinue when baby is over 30” tall or can stand flat-footed in the unit.

I want to alert the manufacturer to change their guidelines for when baby must discontinue using product based on height. This product is already dangerous to a baby 28” tall!

Also, manufacturer MUST PRINT ON THE PRODUCT ITSELF VERY CLEARLY WHEN TO DISCONTINUE PRODUCT BASED ON HEIGHT. (revised guidelines to 28” tall.)

While the 17 pound five month old infant was sitting in the excersaucer the three legs popped out of place from underneath him causing the exceraucer to collapse.

The consumer removed him immediately and there were no injuries sustained. The excersaucer has three plastic legs and three pegs used to adjust the height of the excersaucer.

The excersaucer was used at the lowest height when the peg’s on each leg apparently popped out of place causing the legs to collapse.

The firm was contacted on 3/5/2012 and was advised that they would send out a box from the consumer to mail the excersaucer back for evaluation. The consumer stated that they also offered her a full refund.

My almost 5 month old, 15 pound son was playing in his evenflo Johnny jump up. I inspected everything before putting him in it. After about 5 minutes of play the straps on the hanger pulled out of the seat, causing my baby to fall face first, smashing his face and head onto our tile floors. We were advised to go to the emergency room by his Pcp and so we did so. After hours and a CT scan later, we learned our son was okay. It could have been a lot worse and i will never put my child or any other child i know in this bouncer again! Until the straps are perfected this toy should be recalled! When i bought this toy it didn’t even come with a product recall form. It needs to have one.

The bouncy seat we are using comes apart from the lets and the top. To install the unit you must push down and twist to remove the legs and when the baby bounces on the seat the same things happen if he twists his body while jumping. There is not locking device no snapping snug fit , its only twist and the legs come right off the top of the bouncy seat. It is very unsafe and poor design and my son fell to the ground on Sunday becuase of the poor design. He fits the required age and weight limit and it was accurately put together based on the directions in the box.
