The reports, as CPSC published them
Reports people made to the U.S. Consumer Product Safety Commission. CPSC publishes each one after sending it to the company named, and doesn’t guarantee they’re accurate. A report isn’t a recall.
Each report is the words of the person who made it, as CPSC published them. The company’s comments, where CPSC published any, are set apart as the company’s own.

The product, in the report: Nuna PIPA lite rx
I would like to report a serious safety flaw in the carrier handle bar design of the Nuna Pipa Lite Rx.
Just after a few uses, the following occurred to us. We put our newborn in the Pipa car seat, at home. The Pipa car seat was on the floor, with the carrier handle in the middle position (half way back). We strapped our baby. Then, to lift the car seat, I unlocked the carrier handle and attempted to lift the car seat. My natural expectation is that the handle will just rotate up to 90 degrees, lock and just lift the car seat.
Instead, the center of gravity of the car seat with the child’s weight caused the car seat to violently roll forward, effectively the carrier handle went all the way back, rotating the whole car seat with the baby inside in a truly unsafe and sudden movement. If our baby would not have been strapped when this happened, he would have been catapulted against the floor, head first.
To make things worse, after this happened, I checked the official manual. To my disbelief, there is an explicit warning for this same issue: “[warning sign] Be sure handle is locked in the carry position before lifting child restraint by handle. An unlocked handle could move unexpectedly, causing injury to child.”
This is simply not ok. This is a day to day operation feature of this car seat. Just throwing a warning in the operation manual does not make the issue go away. Having warnings and instructions for things like installing/securing the car seat or base in the car is expected. But having such a design flaw in the day to day handling of the car seat is a very different issue. Note that this happened to us just after a few times of operation. I can just imagine how common this situation is across all Nuna Pipa customers.
There are easy solutions to this safety issue. Just prevent any movement of the handle bar unless both buttons on the sides are pressed. Don’t do it in 0 - 90 - 180 degree increments. Lock the bar at any angle if the buttons on the sides are not pressed. Or add a system that prevents rotation if the handle is carrying certain weight (i.e. it will rotate if you just rotate the handle bar, but it will lock if you lift the seat from the handle bar as it will be carrying the weight of the seat + baby), this won’t even compromise the usability of the handle bar.

The product, in the report: Nuna Pipa lite lx
I have a Nuna Pipa lite LX. When carrying my infant seat, the handle has given out and almost caused me to drop my baby. Handle is properly secure in the upright position when this occurred. It has occurred 4 times since i bought the seat in 2019 (2 with my first baby, 2 with my current baby) i had contacted nuna when it happened the first time with no help and then more recently when it happened again but they said its out of warranty and i had to purchase a replacement base for $150.
