The product, in the report: Chicco keyfit 30 carseat (purchased as part of the travel system).
I have a Chicco keyfit 30 carseat (purchased as part of the travel system). Once I buckle my baby in the carseat, the harness does not stay secure and I can pull the harness loose without pressing the harness release button.

The product, in the report: Child is able to unlock the middle restraint.
I have a concern about the Key Fit 30 car seat. I purchased the item 14 months ago because it was recommended on Consumer Reports. It seemed to work appropriately until my infant turned 14 months. It became quite easy for him to unlock the chest restraint. I plan on purchasing a new car seat but am very upset. I know this is not the most expensive car seat but it was pricey and should last.

Death Made Nov 26, 2018, sent to the company Dec 4, 2018, published by CPSC Jan 29, 2019
The product, in the report: Chicco car seat, no other description given
On July 4, 2018 mom put both children (twins) into separate car seats to take a nap at a friend’s house. Mom recalls buckling both the chest strap and bottom buckle for both children prior to leaving the room. The victim was later found unresponsive by a friend and declared deceased. Friend reports bottom buckle was undone when she was found, and it appeared that she wiggled down to where the chest strap was under her neck. On autopsy, the ME found no signs of trauma and reported that it appeared that the victim died form Asphyxiation due to compression of the neck.

No one hurt Made Oct 3, 2015, sent to the company Oct 15, 2015, published by CPSC Oct 29, 2015
The product, in the report: Chicco KeyFit 30 infant car seat
My mother in law took my almost 5 month old for a walk using our [REDACTED] stroller using the Chicco KeyFit 30 infant car seat with the [REDACTED] adapter. As she was walking down the street, the adapter failed and the car seat tumbled backwards out of the stroller landing on the street. My daughter thankfully was unharmed while she was strapped securely in the car seat.
You can imagine our horror at the incident itself, but after conducting additional research, I see this was a prior issue that was recalled back in 2013. The stroller, car seat, and adapter kit were all purchased new in 2015 not subject to the old recall, but apparently this is an ongoing issue that should be addressed immediately.

Emergency room Made Aug 16, 2014, sent to the company Aug 26, 2014, published by CPSC Sep 10, 2014
The product, in the report: Chicco Keyfit 30
I am writing to make you aware of a malfunction in the [REDACTED] car seat adaptor for its [REDACTED] single stroller. I purchased the single car seat adapter from Babies R Us in Madison, Wisconsin in May 2014 so I could use my Chicco Keyfit 30 with the[REDACTED]. Our baby was born in June 2014. In July 2014 my husband and I placed the adapter in the stroller and then placed the car seat in the adapter to go for a walk with our four week old. We had used the car seat with the stroller several times since our daughter’s birth without incident.
Approximately 50 feet from our apartment the adapter and car seat fell off the stroller (the adapter was still attached to the stroller), hit the concrete pavement, and completely rolled over. Our newborn received scraps to her head and we spent the day in the emergency room so she could be observed to rule out head injury.
We are obviously never going to use the adapter again and believe you should be aware of the malfunction. We believe the adapter was securely placed in the stroller and that the gray buttons on the side had “clicked.” As you will see from the attached photos, their is a piece of elastic cloth that partially covers both holes where the adapter is supposed to click in so that you hear and see a click but in fact the cloth is preventing the adapter from being fully secure. The photos even show an indentation where the adapter pressed against the cloth thereby preventing it from securing. The instruction manual makes no mention of needing to move the elastic to have the adapter fit securely.
We feel the most important thing is for you to be aware of the malfunction so that no other babies get hurt or end up in the emergency room.
