The product, in the report: Baby Jogger Car Seat Adapter for BRITAX and BRITAX B SAFE Chaperone Car Seats with Baby Jogger City Mini GT Single Stroller
My babysitter placed our five month old in our Britax B safe carseat and connected it to our Baby Jogger City Mini GT using the Baby Jogger Car Seat Adapter for Britax. About five minutes into their walk, the carseat fell off of the stroller, landing upside on the sidewalk/grass. My baby was still secured in his seatbelt but landed upside in his carseat and began crying. The babysitter removed baby from carseat to make sure he was okay and did not attempt to put the carseat back into the adapters, instead carried the baby home.
When I came home, we looked at the car seat and tried connecting the car seat to the stroller using the adapters, and it appears that the adapters are not “clicking” into the baby stroller; therefore, they are not secure enough to hold the carseat on the stroller.
The stroller and adapters have only been used a couple of times since purchasing in the fall. The carseat hood was damaged as a result of this incident.

The product, in the report: Car Seat Adapter for Baby Jogger City Mini Stroller
The car seat adapter for our Baby Jogger City Mini stoller, popped off the stroller at the connection points flipping the carseat and my 2-month old baby girl to the pavement.
We have owned the stroller and adapter for 2 1/2 years. We used it with our son when he was an infant without incident. My husband attached the adapter and carseat before our walk. As we were leaving our driveway, the front wheel of the stroller bumped a lawn sprinkler head and the impact was enough to jolt the carseat adapter and attached carseat to the pavement. The carseat flipped forward and landed upside down. I immediately unstrapped my daughter and cradled her screaming little body to my own. I held her tightly rocking and screaming, “Oh my God, Oh my God, that should not have happened, what happened?” We took her to an urgent care pediatric office where she was examined and no noticeable signs of injury are apparent. We are watching her closely for the next 48-hours for any signs of a head injury. This should not have happened.

The product, in the report: Car Seat Adapter for Baby Jogger City Select Stroller
I was using my Baby Jogger City Select with the new Universal Car Seat Adapter bar in place. My daughter was secured in her Chicco Keyfit. She was 6 months old and around 17 lbs at the time of the incident. I was shopping and hear a noise and saw the seat fall down onto the ground. My daughter was crying really bad so I quickly unfastened her from he carseat and held her. I then left the store I was in and confirmed she had hit her head, but not to a point where she needed any medical attention. She was upset and I was able to calm her down after a bit. Upon examination, the carseat adapter bar had snapped in half where the red plastic inserts in the tubing. I contacted Baby Jogger who was able to replace the adapter bar and sent some extra accessories. They did not request the bar.

No one hurt Made Aug 9, 2011, published by CPSC Aug 30, 2011
The product, in the report: The Baby Jogger car seat adapter for Double City Series Strollers
The Baby Jogger Car Seat adapter is designed to allow a Graco SnugRide car seat to be installed on a Baby Jogger City Mini series stroller. However upon installation the car-seat does not seat firmly on the adapter, instead the hinges of the car seat adapter ‘balance’ on the uprights of the car seat adapter, making it very unstable and unsafe. The manufacturer provides a nylon strap to secure the car seat to the adapter, however this does not provide a secure car seat (unlike when it’s used with a car seat base in a car).
I will be returning the product to Baby Jogger, as I don’t want my 3-month old daughter placed in any danger with this product. Similar consumers have had similar problems, as evidenced by the comments on the amazon.com website )http://www.amazon.com/Baby-Jogger-Car-Seat-Adaptor/product-reviews/B00155UGTO/ref=dp_top_cm_cr_acr_txt?ie=UTF8&showViewpoints=1)
