The product, in the report: Graco Pack ‘N’ Play with bassinet attachment
A 3-month-old infant was placed in the bassinet attachment of a Graco Pack ‘N’ Play to sleep. The attachment somehow became unhinged by 1 or 2 of the hooks and was at an angled decline. The infant reportedly slid down the decline and her face was adjacent to the mesh webbing of the side wall. The infant was found dead in this position.
