The product, in the report: Graco Pack ‘n Play Simple Solutions Playard with Changing Mat, Cam. 28.25 x 39.5 x 29 inches
I received the Graco Simple Solutions Pack & Play with Changer as a gift for my baby. The changing table portion of the product is totally tilted and is not safe for babies. My baby is small (6th weight percentile) and can’t roll over yet, but she’s almost fallen off the table several times during a diaper change even though I am standing there. I’m worried about what will happen when she gets bigger, even within the weight limits of what Graco says is acceptable for this product. I emailed Graco to let them know the table tilts but they told me that’s normal and safe. I noticed that many other Graco Pack & Play items come with clip on “feet” to secure the changer properly to the playpen but they said this one doesn’t need it. I’ve had childcare professionals (nannies, doulas, lactation consultants) at my house who have ALL commented that the changer is dangerous due to the tilt.

Ongoing issue with Graco pack and play: mattress will not lay flat, creating a significant ridge and gap between the mattress and playyard.

First aid Made Sep 4, 2024, sent to the company Sep 24, 2024, published by CPSC Oct 8, 2024
The product, in the report: Play yard
The consumer’s 2months old son was on the mesh portion of the play yard, where babies are intended to be and he began kicking the mesh portion, when he sustained lacerations to high right pinky toe. The baby was on the mesh portion for less than 5 minutes, when he began crying. The consumer found that as the baby was kicking his feet, his right pinky toe hit that mesh potion repeatedly and caused the injury. She will treat her son at home, and he does not need to see a doctor.
The consumer used the play yard for her daughter the past year and there was no injury. However, she never used that portion of the play yard with her daughter.
She’s concerned that other moms may experience the same exact type of incident.

The product, in the report: Graco pack and play quick connect portable seat deluxe playard
I had originally purchased a Graco pack and play quick connect with portable seat in 2021 for my son. When he outgrew it we collasped the pack and play per instructions and stored it in our home. In May 2023 I set up the pack and play in anticipation of our daughter. I noticed a big bubble across the width of the mattress. I tried adjusting the mattress and setting heavy books on top to reduce the bubble but nothing has worked. This is no longer a safe space for my baby. No injuries have occurred.

The product, in the report: Pack ‘n Play® On the Go™ Playard Pack ‘n Play® On the Go™ Playard Color: Kaden SKU: SAP_2164119
Received Graco pack and play with defective bubbling mattress that was progressively getting worse. Contacted Graco customer care to assist with the issue, I was told to send the pack and play back because the mattress was out of stock so it could not be replaced. This pack and play is the only safe place that we currently have for our baby to sleep, and the agent I spoke with was made aware of this. They again told me the play has to be dropped off at [REDACTED] and I need to provide a tracking number to the agent via email in order for the replacement pack n play to be sent out. I responded and advised I have nowhere else for our child to sleep safely, and I was met with the same response - one will not be sent to me until I provide a tracking number. I reiterated we have no other safe sleep space currently. Several days later I reached out to customer service again and was told that we need to discontinue use of the pack and play immediately as it’s not safe. I WAS NOT TOLD THIS FOR SIX DAYS EVEN THOUGH THEY REPEATEDLY RESPONDED TO ME. That should’ve been the FIRST thing that I was told by them - our baby was still sleeping in this pack and play! Luckily we have the means that we were able to go and purchase a pack and play made by a different manufacturer. Had we not had the funds to do this, where would our child have slept? And why were we not told at first contact to stop using the pack and play? This is a huge problem. Graco handled this horribly and a child could have been injured because they’re what, greedy over an item that costs them $20 to make? It shouldn’t have been defective to begin with.
As an aside I went and read through Amazon reviews and the mattress bubbling is a known issue yet they continue to sell the play yards with this same mattress. Ridiculous.

The product, in the report: Graco Pack N Play Portable Playard in “Marty” colorway [REDACTED] Product Width: 28.25 in Product Height: 49 in Product Depth: 39.5 in Product Weight: 18.7 lb
We purchased a Graco Pack N Play Playard to use as a temporary crib prior to taking a family vacation. The first time we used the thing was the first night at our rental home and we were horrified the next morning to find that our 5 and half month old baby had red abrasions to both of his cheeks. We felt the bottom sides of the Playard and found the stitching and the mesh side panels to be coarse, hard, and right at face level. Having no other options, we continued to use the playard as a crib but slathered our son with ointment every night. Over the course of just five days his cheeks were rubbed red and raw by this cheese grater of a crib and they’re still red almost two weeks later.
It’s unfathomable that not only is this product still available, but that it’s also the standard option that almost every new parent goes for - it’s got the market cornered somehow despite being dangerous and negligently designed.

The product, in the report: Graco pack n play with nuzzle nest sway seat
I received my Graco Pack n play from my baby registry. My husband and I followed the directions putting it together. Everything is fine aside from the changing area. Babies will roll to one side. This is a huge safety concern! We called Graco to report this, they sent us out new parts and the same problem occurred. I looked online to see if we were doing something wrong, and found that we are one of many having this same issue. Even in the directions where it shows how to put this part together, the problem is evident. the clasps that are supposed to hold the changer up and stable are way too close to the one side that is already holding up the changer. I’m not sure how this was overlooked, but this is a huge problem and major safety concern.

The product, in the report: Play Yard
Caller stated that her 9 month old child was rubbing her face against the netting on the play yard which caused her to receive lacerations on her nose. She was placed in the play yard for only 5 mins when the incident occurred. She took her to her pediatrician for examination, she was not given any medical treatment. She was advised not to touch the child’s nose to prevent an infection.
6/24/13 The manufacturer was contacted through email but no one responded. (6/27/14) The manufacturer informed her that she should return the product for a refund. She requested to speak to a Supervisor who advised that the person she spoke was in charge of that department. Caller feels that the manufacturer should change the design of the netting that caused this incident. She is concerned that the company is not willing to prevent harm to children.
Caller feels that this product is a safety hazard for babies.

The product, in the report: Pack N Play with bassinet and newborn napper
The other day I found a small piece of nylon webbing (approximately 1/2” x 2”) in the bassinet portion of my Graco Pack N Play. It was not sewn into the product properly, if at all. There are supposed to be a total of 4 of them sewn in the bassinet portion of the Pack N Play to attach the “Newborn Napper” to. In mine there are now only 3 sewn in. It concerns me because this small of a piece is a choking hazard and I am hoping this is an isolated incident. I am disappointed in this product, as I felt it was a safe place for my baby to play. I purchased this product new on January 26, 2013 from Babies R Us. I have my receipt, registered my product with Graco and I have also contacted Graco regarding my concern.

The product, in the report: Graco Pack ‘N Play with Cuddle Cove rocking seat The model number for the Graco Pack ‘n Play with Cuddle Cove Rocking Seat is #1780633.
I was using the Graco Pack ‘n Play with Cuddle Cove rocking seat as the instructions and guidelines suggest. I was letting my 6 week old nap in the rocking chair attachment on the play yard. When I went to check on her, I found her still strapped in, but her head turned and face flush against the head support padding. She was not able to breathe. Thankfully she was not injured, but if I would not have checked on her when I did, she would have suffocated.

The product, in the report: Graco Playard model # 9B08CAT
I have a Graco Playard model # 9B08CAT manufactured in 2/2010 and in use since 1/2011 (~16 months). When using the bassinet portion of the playard for my 4 month old son, he began crying like he was hurt. A different cry than usual. I went into the bedroom to check on him and pick him up and he was stuck. I couldn’t physically pick him up. I then realized his head was stuck under the oval wall part of the bassinet and the 4 panel mattress has bunched up (the 4 panels overlapped each by about 1/2 inch - 1 inch) to create a space at the end of the mattress. He was stuck face down into the nylon bottom of the bassinet with the mattress pinning his head against the wall and the oval wall part above his head pinning it down into the nylon floor beside (not on top of) the mattress. I pulled the mattress further away from the wall and was able to maneuver his head out to free him. I am an emergency physician and saw that he was unhurt but also see the potential for this to fatally injure an infant. I wanted to report this so that if there other similar reports, a safety recall could be issued to prevent future injuries. I believe the problem is that the fabric of the mattress has stretched a bit over the 16 months of usage and now allows the 4 boards inside the mattress to overlap a bit which creates space at the edges of the bassinet. Because the bassinet has walls that shape the space into an oval, the oval wall can trap a baby’s head. My mattress also with 16 months of use has bowed a bit also shortening the panels. I have only used my playard in line with Graco’s recommendations.

The product, in the report: Pack N Play / play yard
The consumer stated that his 19 month old son place his hands on the side of the Pack N Play and started leaning backwards. The consumer stated that his son was in the Pack N Play and woke up after his 1 hour nap. The consumer stated that he noticed that his son pull plastic of the area where the hinges are located. The consumer stated that because his son pulled the plastic off, it created a sharp edge on that area. The consumer was concerned that his son could get cut and he covered the edges of the Pack N Play with tape, to hold the plastic together.
The consumer’s son was not injured. The consumer continues to us the Pack N Play.
He contacted the manufacturer and was not able to get a representative on the line to assist him.
The consumer feels that this is a laceration hazard that needs to be investigates.

The product, in the report: Graco silhouette pack n play in Crawford print
I’ve been using a Graco silhouette pack n play in their Crawford print for about a year now. Once my baby was big enough to put him in the bottom portion of the playpen I started having problems. The mattress does not lay properly in the bottom of the playpen and that causes the quilted mattress cover to bunch in the middle. (It seems like that’s an area where the mattress folds for storage.) The bulge in the middle of the mattress is pretty big and could become a suffocation hazard.
I have contacted Graco in November 2011 and they sent me a temporary mattress (mine was on back order) and the temporary mattress did the same thing. I also just received the final replacement last week and it bunches as well.

The product, in the report: Graco Playard - A portable play area that is surrounded with netting for safe play or sleep.
My child was seriously hurt while using a Graco Playard. Her front tooth was ripped out of her mouth, when she had a fall in the playard, and her tooth was stuck in a mechanism that releases the sides of the playard. The mechanism acted as a hook that my daughters tooth got stuck on. I was advised at the hospital that this type of injury could of also broken her jaw, and they are unsure as to what damage there may be to her permanent tooth. My daughter continually suffers from pain from this injury.

The product, in the report: Graco Pack’N Play Playard Model Name: Pack’N Play Model #: 1761363 Date of Manufacture: 01/27/2011
My son’s Graco Pack n’ Play caught on fire on September 12, 2011. Thankfully he was NOT in it and no one was hurt! A light bulb in the room blew and it appears that the shattered glass was hot enough to cause ignition and or maybe it sparked. I contacted Graco and they informed me the following: “The foam in the mattress of all pack n’ plays are fire retardant and meet the Federal Flammability Standards”. Then I asked about the Graco Playard Sheets, which we use to protect the mattress on the Pack’N Play, and Graco informed me that these sheet are NOT fire retardant. It is my believe that the the Graco Playard Sheets are flammable. There is a bed with sheets on it in the same room, next to the Pack’N Play, under the light bulb that blew, and it did not catch on fire. The only thing in the room that caught on fire was the Pack’N Play. Thankfully my son was not in it at the time the incident occurred. The Graco Playard Sheets need to be inspected to prevent future recurrence and or a tragedy.

The product, in the report: Graco Rittenhouse Pack and Play
While using the changing station portion of the Graco Rittenhouse Pack and Play, my infant son’s finger got trapped in the compartment that the baby wipes are contained. This happened while I opened the lid to refill the wipes, my son’s finger wandered into the hinged area and got severely hurt when I closed it. This area should either be located in a different part (not accessible by the infants) or should have a mechanism to cover the door hinge so that this cannot happen.

The product, in the report: Graco play yard 1756464, manufactured 12/24/2009
We purchased a Graco play yard (model 1756464, manufactured 12/24/2009) a few months ago from Burlington Coat Factory. Once our baby was born on 4/7/11, we began using the changing table portion. There is a triangular cutout at each end of the plastic in the changing table. This area is dangerous - our son has caught and scraped hos toes in it. New designs in the store do not have this cutout. We have emailed Graco asking if this part been redesigned. While it appears that the worst that can happen is scraping of the toes/feet, it still shouldn’t be designed so that a child can be injured.

The product, in the report: Graco Pack ‘n Play - pink and gray plaid
My daughter was in a Graco Pack ‘n Play that was empty and not positioned next to anything. She cannot climb yet, as she is only 9 mos old. She leaned over the rail and fell out, head first. She landed on the floor, and sustained bruising to her temple, along with the breath being knocked out of her. I don’t feel that the rails are high enough to safely contain children, and should be looked into as a falling hazard.
