Emergency room Made Jun 10, 2019, sent to the company Jun 24, 2019, published by CPSC Jul 9, 2019
The product, in the report: Parent’s Choice Glass Bottles: Parent’s Choice Glass Baby Bottle with Silicone Sleeve, 9 oz, 3 Count Walmart # 570467252
My 8-month old son was holding a glass bottle (Parent’s Choice brand) on Thursday, June 6, 2019 and the bottle exploded/shattered and cut his hands within seconds of it being given to him. I purchased a 3-pack of the Parent’s Choice bottles a couple weeks ago, and one of the bottles exploded/shattered while sitting in a drying rack on the counter after being washed by hand (it was after a 1-time use). My sister-in-law purchased a second box from Wal-Mart the following week (week of June 3, 2019), and while making a bottle for my son the new bottle broke. In a rush to feed my hungry crying son, she had no choice but to make another bottle (those were the only bottles she had) with the thought of what are the chances of it happening again. This ultimately was the bottle that cut his fingers. He was bleeding uncontrollably and EMS was called to rush him to the hospital. He was examined at the hospital for nerve damage to his hands. The potential for aspiration of a piece of glass by an 8-month-old baby’s was the largest worry, and had me and his Auntie visibly upset. Thankfully, by the grace of God, my baby was cleared to go home several hours later, but keep in mind the alternative that could happen to another mother’s baby. Between the two boxes (3-packs) we had half the bottles explode having never been used. I reported this to the store the day of the incident, and asked that the product be removed from the shelves, but looked today, June 10, 2019 and saw the bottles being advertised for sale on-line. After this incident, I checked the on-line reviews, and saw a woman post on April 13, 2019 that while feeding her baby the bottle shattered for no reason. These two scenarios prove that the bottles are defective and should be taken off the shelves immediately and removed from on-line purchasing options. Please contact me and advise your resolution to this issue. Thank you for your consideration and cooperation in the interests of public safety.

No one hurt Made Mar 25, 2019, sent to the company Apr 2, 2019, published by CPSC Apr 23, 2019
The product, in the report: Glass Baby Bottle With Silicone Sleeve Standard Neck 0+months slow flow 3 pack 9Fl OZ (266mL)
I took out the glass baby bottles with silicone sleeve (standard neck) out of the packaging and was getting ready to wash them. I placed them on the counter top and left them because I had to get my daughter ready for bed as my wife and newborn was in the bedroom having mommy and baby time. All of us where out of the kitchen and heard a pop and then the sound of glass hitting the counter tops and walls. Coming back to the kitchen to see what happened I noticed that one of the 3 bottles had exploded for no reason. The bottles where bought about three weeks ago and where stored in a cupboard inside the kitchen at room temperature.

Seen by a doctor or nurse Made Jun 28, 2012, sent to the company Jul 16, 2012, published by CPSC Jul 30, 2012
The product, in the report: bottles
After using bottles given to us by our adopted daughter’s foster mom, we found out that the ounces marked on the bottles are not accurate. In other words, when you measure 4 ounces in a measuring cup and pour it in the bottle, it shows the water at 4.5 ounces. This means that when mixing formula straight in the bottle, as is common, you have formula that is more concentrated than intended which can be very dangerous for the baby.
