The product, in the report: Storkcraft 3-in-1 activity walker. Blue, white and gray in color with mutlicolored toy attachments.

While my 1 year old was in the Storkcraft 3-in-1 activity walker, the sliding toy attached to the arch along the side of the walker snapped off into two pieces. My 1 year old then choked on one of these pieces, resulting in a blocked airway and vomiting.
