I have one of these busy books that I bought 21 months ago. It looks like theyve updated the design to be safer since.

In the car today, my 3 year old was playing with (this rear facing) and I heard her say mama with an anxious tone. When I asked what was wrong she just made weird noises. I looked in the mirror and she was holding the busy book up in the air, flipping it over quickly. She had put the loop over her head and then accidentally twisted it with flipping the book over. When it was then stuck, she tried to free it by twisting the book but she was panicked, so twisted it the wrong direction making it tighter & tighter.

I was able to pull over & untwist it and get it off her neck but its scary to think that if we had music on & I hadnt realized the anxiety in her tone, it could have been bad. I have a mirror but cant really see her well in it.
