Incident Description: On June 16, 2025, my husband and I were using a Serta Heated Mattress Pad (California King) that I purchased from Amazon on April 2, 2025 (Order #[REDACTED]).

While the mattress pad was in use, the product failed. The mattress pad burned through the fabric, damaged our [REDACTED] mattress, and electrically shocked and burned my husband.

The incident occurred during normal use of the product. The mattress pad was being used according to the manufacturer’s instructions and was connected to a standard household electrical outlet.

We have preserved the mattress pad, both controllers, power cords, product labels, photographs of the damage, photographs of my husband’s injury, and proof of purchase.

The product, in the report: Fleece to Sherpa Heated Blanket by Serta, Twin size with 10 heating settings Model # PNEB F. PFSH.21. Dimensions are 84” L X 77” W x 0.25” H

I purchased a Serta Sherpa heated blanket in Nov. 2021. This morning, 02/03/2023 I was awakened at 0219 hrs. but unsure as to why. As I was repositioning the blanket I touched an extremely hot spot on the blanket. Upon further inspection I found two burnt spots on the blanket with one of the spots burnt completely through. When I checked further, I found that my mattress and sheet were burnt and stuck together. It should be noted that two days prior when changing the sheets, I notice what I thought to be two food stains on the mattress. After this incident that occurred this morning, I took a closer look at the mattress and found six locations where to top of the mattress had melted.

The product, in the report: Serta Damask Stripe 233-Thread Count Electric Heated Mattress Pad, White

When removing mattress pad after using it for several weeks, we noticed the connector from the transformer to the pad was browned and the wiring is charred. The wire itself has nearly broken free. The wire is stranded copper and very few strands remain.

I bought this September 2011. Nothing out of the ordinary was done with this mattress pad but just over a year of use and the cord has melted. This could have started a fire!
