No one hurt Made Jul 28, 2026, sent to the company Jul 31, 2026, published by CPSC Aug 14, 2026
The product, in the report: Liquid Sky 20-Shot Aerial Cake firework (Item # BC6390A, UPC 15244 06390). A multi-shot aerial pyrotechnic device featuring a tie-dye patterned paper wrap (blue, purple, green, yellow). Contains 20 vertical firing cardboard tubes glued into a rectangular block.
I am reporting a critical product stability and structural failure with a Black Cat “Liquid Sky” 20-shot aerial cake (Item BC6390A). On the evening of July 26, 2026, the device was set up on a clean, flat concrete driveway according to all safety instructions. The fuse was lit at arm’s length by the operator, who immediately retreated to a safe distance. After discharging two initial shots vertically, the cake suffered a bottom clay plug failure and side-casing rupture. This caused the unit to instantly tip onto its side and pinwheel across the concrete, firing the remaining 18 shots horizontally across the ground and striking a neighboring vehicle. No personal injuries occurred, but the horizontal blast caused property damage to the vehicle. Complete security camera video capturing the flat setup, proper operator retreat, and lateral blowout has been preserved, along with physical photos showing the blown-out clay plugs and ruptured casing wall.

Emergency room Made Jul 8, 2026, sent to the company Jul 13, 2026, published by CPSC Jul 27, 2026
The product, in the report: Black cat pirate island
While setting off legal fireworks we had a fireball go towards us. We followed all manufacturing instructions for how to safely launch and we were farther away than the recommended distance. The only exit on the box was where the fuse was.

No one hurt Made Jul 6, 2026, sent to the company Jul 9, 2026, published by CPSC Jul 23, 2026
The product, in the report: Gatling Pack Black Cat multi pack with launch tube. I found the product on their website https://blackcatfireworks.com/fireworks/roman-candles/gatling-pack/
We used the Black Cat Gatling Pack for our fireworks show, and it caused a dangerous near miss. The item was purchased at Noise ‘R’ Us Fireworks located at [REDACTED] in Newport, NH. The packaging did not have “This Side Up” arrows, and it did not clearly state it was a Roman candle. Only now in the fine print of the warning am i seeing it say it’s a roman candle which was easily missed. The instructions were very confusing and made it sound like you were supposed to light the item, drop it into the safety tube, and run away. Because the directions were unclear, it was loaded upside down. The firework shot out s3ideways directly at 3 people.

Emergency room Made Aug 24, 2019, sent to the company Sep 3, 2019, published by CPSC Sep 24, 2019
The product, in the report: fuseless firework
Child hurt by Black Cat Torpedo cracker snap that exploded in a small glass container sending shards into his subcutaneous tissues. These fuseless firecrackers are much more potent than the “pop pop” snap noisemakers kids often play with. Recommend age restriction for purchase and increased safety warning labels
