The reports, as CPSC published them

Reports people made to the U.S. Consumer Product Safety Commission. CPSC publishes each one after sending it to the company named, and doesn’t guarantee they’re accurate. A report isn’t a recall.

Each report is the words of the person who made it, as CPSC published them. The company’s comments, where CPSC published any, are set apart as the company’s own.

Made Apr 22, 2013, sent to the company Apr 29, 2013, published by CPSC May 13, 2013

The product, in the report: Worthington 14.1 oz/400 g propane cylinder. Blue, approximately 2-7/8”diameter x 10-1/2” tall

    Worthington Propane Cylinder 14.1 oz/400g

I am writing because of a propane valve leak. I am aware of the recent Worthington cylinder recall; however, this recall does not apply to propane cylinders.

Last night, I attached a 14.1 oz/400g Worthington Propane cylinder to my [REDACTED] 2-Burner Stove with Piezo Ignition.

After using the stove, I disconnected the propane cylinder, which immediately began to leak.

I left the cylinder outside to prevent gas buildup. It continued leaking throughout the night.

No injuries or property damage were caused.

  2. No one hurt

Made Oct 5, 2011, sent to the company Oct 14, 2011, published by CPSC Oct 28, 2011

The product, in the report: 20# propane tank MFG by Worthington Ind

I purchased 3 - 20# propane tanks at Menards in Mansfield, OH on 10/3/11. Manufactured by Worthington Industries, Columbus, OH. MFG date 5/11.

I had all 3 tanks filled on 10/4/11. One of the tanks leaked excessively for the next 24 hours. I went back to the propane seller and they checked it and found the overfill valve had failed and the tanked was dangerously overfilled. These new tanks are suppose to shut the fill off at 20# and 1 of the 3 did not. They bleed it down to the proper poundage but if I had not noticed the hissing sound I could have been in a very serious fire.

I sent a E-mail to Worthington Ind. but I am sure they won’t do anything unless they are made to.

  3. No one hurt

Made Jun 20, 2011, published by CPSC Aug 26, 2011

The product, in the report: Brass Gas Tank Valve

The consumer says he filled the gas tank but it wont work on the bbq grill. He says gas leaks from it posing a fire hazard. The consumer says there is a universal cylinder that is supposed to work on all gas tanks but not when using the grill. He says this is a newer cylinder where he does not need a wrench to spin it. He says he has only had it filled 4 or 5 times. The next time he filled it is when he had a leak in it. he says the local gas company and says they filled it the same way they have been doing and did not look any different. The product is similar to a 1997 recall for the same company under Release # 97-174, however the consumer’s valve is much older.
