The product, in the report: Neon Butane Refill Canister (750 ml)
RE: Catastrophic Valve and Seal Failure Resulting in Flash Ignition PRODUCT: Neon Butane Refill Canister (750mL) APPLICATION DEVICE: Standard Culinary Torch Attachment
INCIDENT DESCRIPTION:
While operating a standard culinary torch attachment mated to a 750mL Neon Butane canister, the canister suffered a catastrophic structural failure at the main interior seal. Rather than maintaining a sealed pressure boundary, the internal stem and seal completely failed, causing the pressurized liquid fuel to forcefully vent backward out of the valve down the barrel of the culinary torch.
The escaping gas immediately ignited, creating a high-velocity, downward-directed rocketstage flash explosion. The intense thermal blast directed entirely onto the operator’s hands, which acted as a deflector pad for the venting fire. The operator had to forcibly disconnect the torch mechanism to cut the primary fuel stream, extinguish the active fire, and submerge the rapidly venting canister into a sink drain under running water to mitigate a total catastrophic room explosion.
INJURIES SUSTAINED:
The operator sustained thermal flash burns across both hands, concentrated heavily in the high-mobility crescent webbing between the thumbs and index fingers, along with the total destruction and singeing of the hair follicles across the right and left forearm.
REQUESTED ACTION:
An immediate engineering audit by the U.S. Consumer Product Safety Commission (CPSC) regarding recent stem design changes in Neon brand canisters.
