The reports, as CPSC published them

Reports people made to the U.S. Consumer Product Safety Commission. CPSC publishes each one after sending it to the company named, and doesn’t guarantee they’re accurate. A report isn’t a recall.

Each report is the words of the person who made it, as CPSC published them. The company’s comments, where CPSC published any, are set apart as the company’s own.

  1. No one hurt

Made May 6, 2026, sent to the company May 8, 2026, published by CPSC May 22, 2026

The product, in the report: 5 lb propane tank

I am reporting a catastrophic structural failure of a DOT-rated propane pressure vessel. On May 3, 2026, I noticed a strong smell of gas and a distinct hissing sound coming from my Flame King 5lb propane tank (Manufacture Date: 02-24). Upon inspection, I discovered a vertical crack approximately 1/4 inch long directly on the side wall weld seam. Submersion testing confirmed gas is actively escaping through the steel weld itself, not the valve or fittings. This indicates a severe manufacturing defect in the weld integrity. 2. What type of product(s) were being used? Brand: Flame King Model: YSN5LB-GAUGE (5lb Steel Propane Tank Cylinder with Gauge) Manufacture Date: 02/24 (stamped on collar) Purchase Date: July 30, 2024 (via Amazon.com) 3. Who was using the product(s)? The product was owned and operated by myself, [REDACTED]. It was located on my residential property in [REDACTED], at the time the leak was discovered. 4. How were the product(s) being used? The tank was used for outdoor camping and overlanding purposes, mounted to an off-road trailer using a standard, industry-recognized stainless steel mounting bracket. The tank was recently filled at a certified propane refilling station ([REDACTED]), where it was not overfilled, as the Overfill Protection Device (OPD) functioned correctly. 5. Were there any injuries that happened or may have happened while using the product? No physical injuries occurred, but a High Potential for Catastrophic Injury was present. 6. How did any injuries occur? While no injuries were sustained, the failure presented a severe risk of fire or explosion (BLEVE). The leak was discovered while the tank was near my vehicle and home. Had there been an ignition source nearby, or had the tank “unzipped” further under pressure while in transit, it could have resulted in a life-threatening explosion or fire. Furthermore, the manufacturers customer support suggested I transport this actively leaking, cracked pressure vessel to a local “exchange” center, which would have put myself and the public at extreme risk during transport.

The product, in the report: Flame King 1 lb refillable propane tank green 8in tall by 3.75 wide

I was using a Flame King refillable 1 lb tank. I was taking the supplied plastic lid off in a garage when the entire brass nipple and valve unscrewed right out of the tank releasing all the propane which was then ignited by a flame. I found myself in a ball of flame which set my gloves and jacket on fire. Thankfully I only received a minor burn to ear and neck. I did strain a hamstring and shoulder escaping the situation and falling to the ground to extinguish the flames.

  3. No one hurt

The product, in the report: Flame King refillable 1lb green propane cylinder

I have purchased the Flame King 1lb refillable propane cylinders with the refill kit. After about 3 years and maybe a total of 2 to 3 refills each on 12 bottles ([REDACTED] has them as well), these bottles have started leaking and spew propane when disconnecting them from appliances and the refill kit. This is not the standard little “pop” you get with the disposable 1lb bottles, they actually spew propane until you get them unscrewed. This presents a fire hazard and we no longer feel safe using them. There is a recall on these, however, the ones we have are newer bottles and are manufactured after the serial numbers affected by the recall. It is my opinion, that the recall should have been for ALL of these bottles, and no longer sold until the dangerous issue with them is fixed!
