No one hurt Made Jan 15, 2014, sent to the company Jan 29, 2014, published by CPSC Mar 14, 2014
The product, in the report: High pressure scuba diving air hoses The air hoses are black braided high pressure scuba air hoses. The hoses measure about one centimeter in diameter and this one is about 32 inches long. These hoses attach to the regulator first stage and are intended for use with the submersible pressure gauge (SPG’s) that indicate the air pressure in the scuba diving tank. This hose was sond by [REDACTED].
Aloha,
I wish to report an incident with one of the 32” HP hoses we purchased from [REDACTED] on 11/15/2012.
It seems that the swage on the ferrule on the gauge end was not tight enough. The hose separated from the fitting as shown in the attached pictures. After installation in the shop, the hose tested (~2,700 psi) satisfactorily with no leaks.
The customer brought the hose back to us stating that when he grabbed his console to check his SCUBA cylinder pressure at the start of a shore dive, the gauge/console separated from the hose with a rather loud noise and traveled some distance. Other divers retrieved the gauge and console boot from the ocean. The diver closed the tank valve and returned to shore.
No one was injured...surprised and a little scared by the noise maybe. Cylinder/reg: [REDACTED] HP 120 cylinder at approx 3,500 psi with a [REDACTED] [REDACTED] 5000 regulator.
I believe that this is a manufacturing defect. The hose should have not separated from the insert swaged outer body. There is no evidence or any tears, rips, abrasion or any other damage to the hose or fittings. the hose is practically new wit honly 2-3 dives on it. Should last for years.
This same style of hose was part of an earlier recall 13-029 (High Pressure Scuba Diving Hoses Recalled by Innovative Scuba Concepts, and [REDACTED] Due to Drowning Hazard)
[REDACTED] has custody of the parts shown in the pictures. Please let me know what comes next.
Mahalo and Aloha,
[REDACTED] [REDACTED]
