  1. No one hurt

The product, in the report: HOG ZENITH SECOND STAGE BLACK http://edge-gear.com/index.php/products/regulators/product5.html

Manufacturer: Edge-Hog Website: http://edge-gear.com Product: HOG Zenith Second Stage Black Product Website: http://edge-gear.com/index.php/products/regulators/product5.html

Product Failure: The purge cover detaches from the screw-on purge cover. The consequence is that the operation of the purge function of the regulator may be stuck in the full ON position. The result of this would be a rapid loss of gas supply to a SCUBA diver. Death or serious injury may result.

History: I first experienced this problem on two occasions in October 2015. I returned the second stage purge cover/actuator to the manufacturer who examined it. The manufacturer returned the covers to me with the explanation that there was no defect.

On April 30, 2016 I again experienced the defective operation of the second stage purge cover while on a cave diving trip to the cenotes located near Playa del Carmen, Mexico. Cave diving is inherently dangerous. The safe operation of diving equipment while caving diving is a life or death matter.
