The posting states that no accidents or deaths have been reported in the U.S.
On November 6, 2004, [REDACTED] died in a climbing accident in the Shawangunks of New York. Failure of a Cassin Joss Cam was the major contributing factor to the fatality.
I conducted the accident investigation and notified both Sierra Trading Post and CAMP/Cassin. I had extensive followup communications with CAMP/Cassin.
