  1. No one hurt

Made Jul 26, 2017, sent to the company Nov 6, 2017, published by CPSC Nov 21, 2017

    The product, in the report: Model # WM42033 , 3 swings, 1 glider, 1 slide, 1 see-saw

Purchased Flexableflyer swing set model # WM42033-061013 from Wal Mart , [REDACTED], on 12-14-2013. Was not told that this model number was on a recall for the see-saw seat at the time. This unit should not have been sold by Wal Mart at that time without the problem corrected. Called recall phone number/ service number at Troxel corp. for this recall and was told recall was from 2012 through 2013 for this swing set/ see-saw. Why then is this not covered anymore? July 17, 2017 my 3 yr old granddaughter attempted to use this see-saw with her 2 yr old brother and the handle broke on her side and she fell off, almost catching her fingers in the crack opening that would have pinched and cut her fingers badly. At that moment took them off of the see-saw and removed the plastic seat. Went on-line to the Flexableflyer web site and noticed the recall on this set.

The product, in the report: Flexible Flyer swing set Fantastic Playground model 42126.

Flexible Flyer swing set Fantastic Playground model 42126 was purchased at Paducah Ky Walmart store#0431 on September 14, 2013. We assembled the swing set the afternoon of the 14th and the first use was Sunday September the 15th. After it’s first use, both see saw seats broke, both bolts that hold the seats broke through the seat. Today is September 24th and I decided to email the company to see about getting replacement seats and seen where there was a recall. When was this recall and if before September 14th, why did Walmart sell me this product?

The product, in the report: metal swing set with see saw, 3 swings, trapeze bar, glider and slide The model number that I found on the top of the swing is:WM42126-0312

I would like to report 3 separate incident/safety concerns regarding the Flexible Flyer Fantastic metal swing set. The swing was purchased as a Christmas gift for my two nephews who at the time were 2 and 5. They are now 3 and 6. The swing for purchased on 11/30/2012 as a Christmas gift. On Christmas Day, my 5 year old nephew sat on the see saw seat and it broke. He sustained no injuries at this time. The seat was replaced with the provided replacement parts and within three weeks, he sat on it again and the seat sheared off at the base. Once again, no injuries were sustained but he was clearly upset over the incident. At this point, myself and my brother, decided that we would not replace the seats for a third time and the boys were no longer to play around the see saw area. One month later, my 3 year old nephew went to sit in the middle swing. When he did, the swing seat broke away from the chain. He fell to the ground and sustained only a cut to his arm with a few scratches. Since there are two other swings on the set, we decided to not replace the middle swing. At this point, I began obtaining images of the damaged areas of the swing for my own records. Well, today was the final straw. My 5 year old nephew was swinging on the glider by himself under the supervision of myself, his mother and grandmother, when the glider arm broke from the bolt that held it in place. Upon inspection, the bolt was broken in half. My nephew fell to the ground sustaining scrapes to his hands and elbows. It is a miracle he did not break any bones or obtain a head injury. After this, the swing is officially off limits to everyone. I have contacted the company with images sent as well. I have decided to fnally file a report, because according to their website, the only recall on that particular swing, is the see saw seat. At this point, the see saw seat is the least of their worries. The broken glider and broken swing seat could cause far worse injuries. From day one, no one has been allowed on the swing without supervision. My nephews are of normal size and height for their age with no abuse of the equipment made by them. The point that I am trying to make, is that they are only allowed to sit there behinds in the swing seats and the glider. They have never been allowed to hang from the swing or use it in any other way besides the way it was intended.

The product, in the report: Flexible flyer triple fun11. Model number42560t. Metal swing set

Teeter totter broke with handle in my babies hand on first lift. She fell flat faced into the ground missing the cross bar by less than 1/2 inch. She was not seriously hurt, but could have knocked out teeth and bashed face. This was brand new never been used birthday gift for a 2 year old 30 lb baby. Just picked up from Walmart on sat August 11 and assembled, she tried it out Sunday when it broke. The ladder from the slide also should be looked into, because the steps are so weak they bend under her weight. As this was a birthday gift the whole incident was video taped.

The product, in the report: Flexible Flyer Swing Sets - Triple Fun II #42560

We bought the Triple Fun II #42560 back in Feburary as a present for my son on his birthday. One of the seats on the see-saw broke alone and we tried to fix it. Then last Sunday my son was playing on the one that was not broken yet and while he was up the seat broke, he fell down and hit his face with the metal piece that supports the see saw.

The product, in the report: Flexible Flyer Swing Set....I believe it’s the Triple Fun II

We purchased the Flexible Flyer swing set in at the end of March. Within 2 weeks of purchase both of the see-saw seats broke where the bolts fasten. There were no injuries. I finally got around to contacting the company earlier this week for replacement parts. I’m waiting to hear back from them after providing all the documentation they requested.
