Each report is the words of the person who made it, as CPSC published them. The company’s comments, where CPSC published any, are set apart as the company’s own.

  1. No one hurt

Made Nov 30, 2023, sent to the company Dec 4, 2023, published by CPSC Dec 18, 2023

The product, in the report: Eemax tankless hot water heater. Model Number: AP126480 EFD N4. Serial Number: 2878173. Purpose is to provide warm/tepid water to the emergency eyewash/shower station.

Eemax tankless hot water heater exploded. It is not clear why exactly. Something failed inside the cabinet which then filled up with water. When that level got the 480V power supply, it shorted and exploded with enough force to bend the metal door (the door has a gasket). No injuries as the water heater is in a room that is not usually occupied. But if someone had been it that room, there would be injuries to report. Model # is AP126480 EFD N4. Serial # is 2878173. Eemax is aware of this and has assigned this a case # of [REDACTED]. The purpose of the hot water heater is to provide warm (tepid) water to the emergency eyewash/shower station. The building is used for research and there is a requirement for the eyewash/shower.
